Zusammenfassung

Paragangliome der Kopf-Hals-Region – im klinischen Alltag meist unzutreffend als „Glomustumoren” bezeichnet – sind seltene Tumoren, die in der Regel eine bildmorphologische Anhiebsdiagnose erlauben. Sie kommen an 4 Prädilektionsstellen vor: in der Karotisgabel, im parapharyngealen Verlauf des N. vagus, um das Foramen jugulare und in der Paukenhöhle. Je nach Lokalisation und Klinik werden sie früher oder später entdeckt, sodass sie bei Diagnosestellung wenige Millimeter oder bis zu mehrere Zentimeter groß sein können. Diagnostische und vor allem therapeutische Probleme stellen sich bei multiplem Auftreten im Rahmen von genetischen Erkrankungen.

Die Bilddiagnostik ist entscheidend für Diagnosestellung, Beschreibung der Ausdehnung und Nachsorge. Wegen ihres Gefäßreichtums sind vor allem die vagalen und jugulären Paragangliome eine Herausforderung nicht nur für den Operateur, sondern auch für den interventionellen Neuroradiologen. Eine superselektive präoperative Embolisation kann darüber entscheiden, ob ein Paragangliom operabel ist oder nicht. In Einzelfällen macht die Tumorinfiltration der A. carotis interna präoperativ die Durchführung einer Ballontestokklusion erforderlich.

Abstract

Paragangliomas of the head and neck region – often incorrectly termed „glomus tumors” in everyday practice – are rare neoplasms that allow a radiologic diagnosis at first glance. They are found at four predilection sites; In the carotid bifurcation, at the parapharyngeal course of the vagal nerve, around the jugular foramen and in the tympanic cavity. Depending on their location and clinical manifestation they are diagnosed early or late. Therefore, their size may vary between few millimeters and several centimeters. Special problems arise when the tumors occur at multiple locations in the setting of genetic disease.

Imaging is essential for establishing a diagnosis, for the description of their extension and for follow-up. Because of their high degree of vascularisation especially vagal and jugular paragangliomas are a challenge not only to the surgeon but also to the interventional neuroradiologist. Super-selective preoperative embolization can decide whether a tumor is operable or not. In individual cases infiltration of the carotid necessitates a temporary balloon test occlusion.

• Paragangliome der Kopf-Hals-Region sind für den Erfahrenen eine radiologische Blickdiagnose. Die vagalen und karotidalen Paragangliome zeichnen sich neben ihrer klassischen Lokalisation durch starken Gefäßreichtum aus. Dieser führt in der dynamischen CT oder MRT zu einer raschen Kontrastmittelaufnahme. MR-tomografisch sind die Tumorgefäße als Signalauslöschungen zu identifizieren und führen zum typischen „Pfeffer-und-Salz-Muster”. Die jugulären Paragangliome wachsen um das Foramen jugulare, von wo aus sie in die angrenzenden Räume einbrechen können. Die tympanalen Paragangliome sind eher umschriebene Geschwülste, die ihren Ausgang von der medialen Paukenwand nehmen.

• Der Nachweis von mehr als einem Paragangliom bei einem Patienten und das Auftreten eines Tumors vor dem 50. Lebensjahr erfordern die Erhebung einer detaillierten Familienanamnese und ggf. eine genetische Untersuchung.

• Während bei den tympanalen und karotidalen Paragangliomen eine präoperative Embolisation nicht immer zwingend erforderlich ist, kann bei vagalen und jugulären Paragangliomen eine gründliche Embolisation darüber entscheiden, ob ein Tumor operabel ist oder nicht. Die Embolisation hat das Tumorparenchym zum Ziel und muss selektiv in den Tumor erfolgen. Ein operativer oder interventioneller Verschluss von Tumorfeedern macht eine spätere Embolisation unmöglich und ist daher zu unterlassen.

• Bei Paragangliomen, die die Karotis umwachsen, ist eine temporäre Ballontestokklusion erforderlich, damit das hämodynamische Risiko einer permanenten Ausschaltung des Gefäßes abgeschätzt werden kann. Die präoperative Einlage eines Stents kann das schonende Präparieren des Tumors in der Gefäßwand erleichtern.

• Wichtige Differenzialdiagnosen sind Lymphome (karotidale Paragangliome), neurogene Tumoren (vagale Paragangliome), Metastasen, Hämangiome, Heffner-Tumoren (juguläre Paragangliome) oder ein ektoper Karotisverlauf (tympanale Paragangliome).
