Schlafapnoe

Zusammenfassung

Das obstruktive Schlafapnoesyndrom (OSAS) ist durch repetitiv auftretende, mehr als zehn Sekunden dauernde Atemstillstände im Schlaf charakterisiert. Die Apnoen werden häufig durch Weckreaktionen beendet. Aus der gestörten Schlafarchitektur resultieren u. a. Tagesschläfrigkeit, Müdigkeit und Erschöpfung. Diese Symptome wirken sich auf Beruf und Familie und letztendlich auf die Lebensqualität negativ aus. Die PatientInnen klagen über Konzentrationsschwierigkeiten, Gedächtnisstörungen und berichten über erhöhte Reizbarkeit und Albträume. Träume von OSAS-Patienten sind gewaltsam und angstbesetzt. Die Beschwerden ähneln in vielen Bereichen denen einer Depression und können damit verwechselt werden. Durch die Anwendung eines positiven Überdruckes in den Atemwegen (CPAP). kann eine Verengung der pharyngealen Atemwege verhindert werden. Innerhalb kurzer Zeit, meistens schon nach der ersten Behandlungsnacht, verschwindet die Tagesschläfrigkeit. Unangenehme Träume werden während der CPAP-Therapie nicht mehr erinnert. Unter CPAP wurde eine signifikante Änderung des Depressionsscores bei OSAS-Patienten erzielt, insbesondere bei mittel- und schwergradiger Ausprägung. Differenzialdiagnostisch sollte bei Erschöpfungszuständen, Depression und Albträumen auch an ein Schlafapnoesyndrom gedacht werden.
