Wie nachhaltig sind die frühfunktionellen Vorteile von minimalinvasiv implantierten Hüfttotalendoprothesen? – Ergebnisse einer prospektiv randomisierten Studie zu Effekten der Gangrehabilitation

Zusammenfassung

Studienziel: Ziel dieser prospektiv randomisierten Jahresstudie ist die Evaluierung der frühfunktionellen Vorteile aus der Gangrehabilitation nach minimalinvasiver Versorgung mit einer Hüft-TEP.

Methode: 21 PatientInnen erhielten eine Hüft-TEP über den anterolateralen minimalinvasiven Zugang modifiziert nach Watson-Jones, 20 PatientInnen über den transglutealen Standardzugang nach Bauer. Mittels Ganganalyse (BRK) wurden die Effekte der Gangrehabilitation prä- und postoperativ nach 6, 13, 27 und 47 Wochen mit einer Seniorengruppe verglichen. Die Vergleichbarkeit der Ganggeschwindigkeiten wurde durch Vorgabe von Schrittfrequenzen mittels Metronom (70, 90, 110/min) erreicht.

Ergebnisse: Die frühfunktionellen Vorteile der MIS-Patienten sind bezüglich der Wiederherstellung der Symmetrie in den Stützzeiten und in der mechanischen Belastung auf der operierten Seite „beständig”. Trotzdem erweisen sich die Defizite in der Lastübernahme gegenüber der Kontrollgruppe nach ca. einem Jahr als manifest. Die „Rückstände” bei den Standard-Patienten p. o. zur 6. und 13. Woche relativieren sich am Ende der Studie, der Vergleich präoperativ zur 47. Woche zeigt aber im Gegensatz zur MIS-Gruppe nur geringe Fortschritte.

Schlussfolgerung: Die Ergebnisse der Studie sind bei größeren Stichproben und kontrollierter „Aufarbeitung” des pathologischen Ganges in der AHB zu verifizieren. Für Längsschnittanalysen ist ein standardisiertes methodisches Vorgehen mit vergleichbaren Ganggeschwindigkeiten unabdingbar.

Abstract

Aim: The aim of this prospective randomized study is to evaluate the early functional advantages in gait rehabilitation after minimally invasive total hip replacement (THR).

Method: Twenty one patients underwent a THR using a modified minimally invasive anterolateral Watson Jones approach and twenty patients got a hip replacement using the transgluteal Bauer approach. The effects in gait rehabilitation were measured by gait analysis and compared to healthy subjects preoperatively as well as 6, 13, 27 and 47 weeks postoperatively. The comparison of the walking speed is achieved by given step frequencies using a metronome.

Results: The advantages of the minimally invasive technique in the re-establishment of the symmetry in single limb stance time and in mechanical load are more constant than expected. However the deficits still exist one year postoperatively. The differences between MIS and standard approach, at week 6 and 13 postoperatively, decrease one year after surgery. As opposed to the MIS group the comparison of week 47 in relation to the results prior to the surgery shows only slight improvements.

Conclusion: The results of this study should be verified by the investigation of the pathological gait pattern and with a larger population. For longitudinal analyses a standardized methodical investigation with comparable walking speed is essential.
