Zusammenfassung
Schmerzen sind nicht nur das Leitsymptom rheumatischer Erkrankungen, sondern auch das Hauptleiden der meisten Patienten mit rheumatischen Erkrankungen. Schmerzmessung hat somit eine große Bedeutung bei der Verlaufkontrolle der Erkrankungen und Beurteilung des Therapieerfolges. Zur Schmerzmessung werden sowohl in klinischen Studien als auch im praktischen rheumatologischen Alltag einfache ein-dimensional messende Instrumente wie Numerische Rating Skala oder Visuelle Analog Skala eingesetzt, die Vorteile bei der Messung der Schmerzintensität gegenüber Verbalen Rating Skalen haben. Beim Einsatz unterschiedlicher Methoden zur Schmerzmessung müssen Faktoren wie mentale Einschränkungen oder Demenz bei der Auswahl berücksichtigt und möglichst Instrumente eingesetzt werden, die für die untersuchten Erkrankungen und Patientengruppen validiert sind. Die genauere Erfassung chronisch rheumatischer Schmerzen mit ihrem Einfluss auf und Zusammenhängen mit physischen, psychischen und sozialen Faktoren und Funktionen erfordert aufwendigere mehrdimensional messende Instrumente. Aus verschiedenen erörterten Gründen werden diese Instrumente nicht routinemäßig eingesetzt, obwohl sie richtig eingesetzt über den diagnostischen und dokumentierenden Aspekt hinaus auch therapeutischen Nutzen bekommen können.
Abstract
Pain is the leading symptom and as well the main complaint of most patients with rheumatic diseases. Thus, assessment of pain is essential for monitoring disease activity and therapy response. Pain can be assessed with one-dimensional tools such as numeric rating scales or visual analogue scales. Both are more powerful in detecting changes in pain intensity than a verbal categorical rating scale and are widely used in clinical trials as well as in daily routine care. Any assessment of pain must take into account other factors, such as cognitive impairment or dementia, and assessment tools validated in the specific disease and patient groups being studied. Chronic rheumatic pain assessment and its impact on physical, emotional, and social functions requires more sophisticated multidimensional qualitative tools. Due to various reasons their use is not established in daily clinical rheumatology although they demonstrate therapeutic impact if used appropriately.
