Proximale knöcherne Integration des zementfreien Vektor-Titanschafts in der Hüftendoprothetik. Mittel- und langfristige Ergebnisse

Zusammenfassung

Studienziel: Das Ziel dieser Studie ist die Ermittlung von mittel- und langfristigen Ergebnissen – sowohl klinisch als auch radiologisch – mit dem zementfreien Vektor-Titanschaft unter besonderer Berücksichtigung der Fragestellung, inwieweit das Prinzip des proximalen Kraftflusses und damit die Vermeidung eines „stress shielding“ am proximalen Femur durch das Prothesendesign verwirklicht wird. Material und Methoden: Im Zeitraum vom 10. 10. 1996 bis 18. 05. 2000 wurden in der Orthopädischen Klinik Wichernhaus am Krankenhaus Rummelsberg 432 Vektor-Titanschäfte bei 348 Patienten implantiert und die Ergebnisse im Rahmen einer retrospektiven Kohortenstudie radiologisch wie klinisch ausgewertet. Der Vektorschaft ist ein Titangeradschaft zur zementfreien Verankerung. Er weist proximal ein hohes Volumen auf in Form eines 3-dimensionalen Konus mit Längsrippen, was eine hohe primäre Verankerung in der Metaphyse bewirkt. Ergebnisse: Nach einer mittleren Standzeit von 7,5 Jahren (Minimum 0,1 Jahr, Maximum 11 Jahre, Standardabweichung 2,4) betrug die Überlebenswahrscheinlichkeit des Schaftes 99,08 %. Eine Prothese musste wegen einer periprothetischen Fraktur bei adäquatem Trauma gewechselt werden, eine weitere nach 9 Monaten wegen einer septischen Lockerung, eine 3. nach 14 Monaten wegen Verdacht auf Low-Grade-Infektion, der sich allerdings intraoperativ nicht bestätigte, und nach 75 Monaten ein 4. Schaft nochmals wegen einer periprothetischen Fraktur. Unter Berücksichtigung des Scores nach Merle d'Aubigné [15] ergab sich klinisch in 254 Fällen (58,8 %) durch die Operation eine sehr gute und in 130 Fällen (30,1 %) eine gute Verbesserung. In 33 Fällen (7,6 %) ergab der Score-Wert eine mäßige Verbesserung und in 15 Fällen (3,5 %) zeigte er keine Verbesserung an. Radiologisch war bei 421 Fällen die Knochenstruktur unauffällig, das entspricht 97,5 % der Fälle. Am proximalen Femur und im Bereich der Trochanteren zeigte sich keine Atrophie. Im Gegensatz dazu ergaben sich auf Höhe der Resektionsebene und der lateralen Prothesenverankerung im Trochantermassiv Strukturverdichtungen und eine sichtbare Ausrichtung der Knochentrabekel auf die Prothesenoberfläche als Ausdruck einer direkten Implantat-Knochen-Verbindung, die auch nach langen Beobachtungszeiten unverändert erhalten blieben. Veränderungen im diaphysären Schaftverlauf ergaben sich nicht. Schlussfolgerungen: Der zementfreie Vektor–Titanschaft zeigt im mittel- und langfristigen Verlauf eine niedrige Lockerungsrate bei sicherer metaphysärer Verankerung und Erhalt der Knochenstruktur am proximalen Femur ohne Veränderungen am diaphysären Schaftverlauf.

Abstract

Objective of the Study: The objective of this study is to establish the medium-term and long-term clinical and radiological results with the cementless Vektor-Titan stem. Special consideration has been given to evaluate the extent to which the implant design achieves proximal stress transfer, thus avoiding stress shielding in the proximal femur. Material and Methods: From October 10, 1996, to May 18, 2000, 432 Vektor-Titan stems were implanted in 348 patients in the Wichernhaus Orthopaedic Clinic at Rummelsberg Hospital. The results were evaluated clinically and radiologically within the scope of a retrospective cohort study. The Vektor stem is a titanium straight stem designed for cementless fixation. It has a high proximal volume in the form of a three-dimensional taper with longitudinal ribs. This design achieves strong primary fixation in the proximal metaphysis. Results: The survival probability of the stem was 99.08 % after a mean time of 7.5 years after surgery. One implant had to be replaced due to a traumatic periprosthetic fracture. Another had to be replaced after 9 months due to septic loosening, and a third after 14 months due to a suspected low-grade infection which, however, was not confirmed intraoperatively. The fourth stem required replacement after 75 months, again due to a periprosthetic fracture. Based on the Merle d'Aubigné score [15], surgery led to very good clinical results in 254 cases (58.8 %) and to good improvement in 130 cases (30.1 %). In 33 cases (7.6 %) there was a moderate improvement in the score, and in 15 cases (3.5 %) there was no improvement. Bone structure was radiologically normal in 421 cases (97.5 % of all cases). No signs of atrophy were found in the proximal femur and the trochanters. On the contrary, there was evidence of an even denser bone structure at the level of the osteotomy and the lateral implant fixation within the greater trochanter. There the trabeculae were visibly aligned towards the surface of the implant, indicative of direct biological fixation at the bone-implant interface. These findings remained unchanged over long observation periods. There were no changes along the diaphyseal stem. Conclusions: Medium-term and long-term results with the cementless Vektor-Titan stem show a low rate of implant loosening with stable metaphyseal fixation and preservation of bone structure in the proximal femur and the absence of changes along the diaphyseal stem.
