Zusammenfassung
Auch wenn die Aufmerksamkeitsdefizit- / Hyperaktivitätsstörung (ADHS) nunmehr als eine überwiegend kognitive Störung beschrieben wird, konnte bislang keine Verbindung zu einer spezifischen neurologischen Schädigung im Gehirn aufgezeigt werden. Die Diskussion hinsichtlich der neuronalen Grundlagen zielt neben den im Vordergrund stehenden Störungen der Verarbeitungsverbindungen des präfrontalen Kortex auch auf Normabweichungen innerhalb des prämotorischen Kortex sowie subkortikale Auffälligkeiten. Die neuropsychologischen Auffälligkeiten der ADHS lassen sich zusammenfassend durch Defizite in kognitiven Kontrollprozessen charakterisieren, die Aufmerksamkeit und die höheren kognitiven Funktionen, die sogenannten Exekutivfunktionen, vermitteln. Vor dem Hintergrund der vordergründig beeinträchtigten Fähigkeit zur Aufrechterhaltung der Daueraufmerksamkeit (Vigilanz) konnte in einer eigenen experimentellen Untersuchung neben einer spezifischen Beteiligung der prämotorischen Aufmerksamkeitsfunktionen an den Fehlfunktionen der adulten ADHS ein Ausgleich einer präfrontal gelagerten und als gestört betrachteten exekutiven Funktion gezeigt werden. Wenngleich das neuropsychologische Erscheinungsbild der ADHS in seiner Komplexität zum Ausdruck gebracht wird, kann eine empfohlene Vorgehensweise zur Abklärung der klinischen Diagnose ADHS im Erwachsenenalter zusammengefasst werden.
