Conclusion

A giant epidermoid cyst of Meckel's cave with multicompartmental extension is a challenging entity. Preservation of neural and vascular structures in such epidermoid cysts is the goal of surgical strategy. A near total safe resection is usually sufficient to achieve improvement of the symptoms without additional morbidity.
