Salvage Surgery of the Limb with Severe Pseudoamniotic Band Syndrome: Case Report and Literature Review

Abstract

Pseudoamniotic band syndrome is a rare iatrogenic complication of twin-to-twin transfusion syndrome treated with foetoscopic procedures. We report a severe pseudoamniotic band syndrome in the recipient monochorionic diamniotic twin pregnancy with twin-to-twin transfusion syndrome following a selective foeticide procedure. A male newborn with a severe circumferential amniotic band in the left leg was treated by single-stage excision of the ring and arterio-venous decompression. No complications were encountered. A microsurgical approach to improve the circulation together with ring excision may be useful in some cases.

Zusammenfassung

Das pseudoamniotische Schnürring-Syndrom ist eine seltene Komplikation des Zwillings-Transfusions-Syndrom bei fetoskopischen Verfahren. Wir berichten über ein schweres pseudoamnio­tische Schnürring-Syndrom nach selektivem fetocidem Verfahren. Bei einem männlichen Neugeborenen mit einem zirkumferenten Schnürring des linken Unterschenkels wurde eine einzeitige Schnürringexzision mit mikrochirurgischer arterio-venöser Dekompression durchgeführt. Komplikationen sind keine aufgetreten. Ein mikrochirurgischer Ansatz zusätzlich zur Schnürringexzision kann in ausgewählten Fällen sinnvoll sein.
