Diagnostik, Prophylaxe und Therapie bei neurochirurgischen Patienten mit epileptischen Anfällen

Kernaussagen

• Frühanfälle bedürfen nicht grundsätzlich einer dauerhaften antikonvulsiven Medikation.

• Bei erhöhtem Anfallsrezidivrisiko oder einer gesicherten Epilepsie sind Lamotrigin und Levetiracetam die Antikonvulsiva der ersten Wahl.

• Ziel der antikonvulsiven Medikation sind Anfalls- und Nebenwirkungsfreiheit, wobei die Priorität im Verlauf der Erkrankung sich ändern kann.

• Eine regelmäßige Medikamenteneinnahme sowie das Meiden von Provokationsfaktoren sind Voraussetzung für eine erfolgreiche Epilepsietherapie.

• Sozialmedizinische Folgen der Epilepsieerkrankung, wie Kraftfahrzeugtauglichkeit, Schwerbehinderung, sind zu berücksichtigen.
