Zusammenfassung
Hintergrund: Therapiekonzepte eines oligometastasierten, nicht kleinzelligen
Lungenkarzinoms hatten sich in der letzten Dekade von der palliativen auf die mit kurativer
Intention verändert. Die Bedeutung der chirurgischen Resektionstechniken in dieser multimodalen
Therapie bleibt weiter in der offenen Diskussion.
Patienten und Methoden: 34
Patienten mit 1 oder 2 Metastasen wurden vom Januar 1998 bis Januar 2013 retrospektiv
untersucht.
Ergebnisse: Das mittlere Patientenalter war 59,7 ± 10,1 Jahre. Die
Männer/Frauen-Verteilung war 20 vs. 14. Das Adenokarzinom war mit 58,8 % der häufigste
Karzinomzelltyp. Die synchrone Metastasierung lag in 15, die metachrone Metastasierung in 19
Fällen vor. Die singuläre Metastasierung betraf 27 Patienten. Zwei Metastasen wurden in 7 Fällen
nachgewiesen. Am häufigsten wurden zerebrale (58,8 %) und pulmonale (23,6 %) Metastasen
diagnostiziert. Die Primärtumorresektion war als R0 in 20 und als R1 in 2 Fällen möglich. Die
allgemeine Überlebenszeit, die lokale und systemische Tumorfreiheit betrugen jeweils 40, 38 und
25 Monate. Das 5-Jahres-Überleben, die lokale und systemische Tumorfreiheit nach 5 Jahren waren
entsprechend 29,2, 26,9 und 16,5 %. Die Behandlungsstrategie unter Einschluss der chirurgischen
Techniken sowohl am Primärtumor als auch der pulmonalen Metastasen in Kombination mit einer
Lymphknotendissektion sowie die operative Resektion der zerebralen und extrazerebralen
Metastasen waren unabhängige prognostische Faktoren für
Langzeitüberleben.
Schlussfolgerung: Die chirurgische Therapie ist bei
Vorliegen eines oligometastasierten nicht kleinzelligen Lungenkarzinoms sowohl im Bereich des
Primärtumors als auch für die Metastasenlokalisation möglich und stellt eine effektive Option
der multimodalen Therapie bei selektierten Patienten dar. Die Lymphknotendissektion sollte ein
wichtiger integraler Bestandteil der chirurgischen Therapie bleiben.
Abstract
Objective: The therapeutic strategies for oligometastatic non-small cell lung cancer have
changed over the last decade from palliative to curative intent. The role of surgery in this
multimodal treatment in selected patients remains a subject for open
discussion.
Methods: Data of 34 patients with one or two metastases treated
from January 1998 to January 2013 were retrospectively analysed.
Results: The mean
age was 59.7 (± 10.1) years. The male vs. female ratio was 20 vs. 14. Adenocarcinoma was the
most common histological type (58.8 %). The synchronous metastases were present in 15 patients,
the metachronous in 19 patients. Single metastases were present in 27 patients, two metastases
in 7 patients. The most frequently involved organs were brain (58.8 %) and the lungs (23.6 %).
The primary tumour resection was achievable in 20 patients as R0 and in 2 patients as R1. The
median overall survival, the local and the systemic disease-free survivals in the entire group
were 40, 38 and 25 months, respectively. The 5 year overall survival, the 5 year local and
systemic disease-free survivals were 29.2, 26.9 and 16.5 %, respectively. The treatment
strategies including surgery for primary tumour as well as for pulmonary metastases site,
combined with the lymph node dissection and the resection of the extracerebral and cerebral
metastases, were identified as independent prognostic factors for long-term
survival.
Conclusion: Surgery in oligometastatic non-small cell lung carcinoma
is feasible for primary tumour and for metastases. It is an effective option in the multimodal
treatment in highly selected patients. The lymph node dissection should remain an important
integral part of the surgical treatment.
