Schilddrüsenszintigrafie – Normalbefunde, Fehlerquellen und Pitfalls

Zusammenfassung

Dem Anwender müssen die Limitationen bekannt sein, die sich aus dem unterschiedlichen pharmakologischen Verhalten von ^99mTc-Pertechnetat im Vergleich zu Radioiod ableiten. Die meisten Fehler treten bei der differenzialdiagnostischen Abklärung von Schilddrüsenknoten auf. Die Dignität kalter Knoten wird zu häufig histopathologisch abgeklärt, die Diagnose indifferenter Knoten wird zu häufig gestellt und warme sowie heiße Knoten repräsentieren keinesfalls immer eine unifokale Autonomie. Große Unsicherheit besteht bei der Abklärung der funktionellen Relevanz einer Autonomie.

Abstract

The user must be familiar with limitations due to the different pharmacological behavior of ^99mTc-pertechnetate and radioiodine. Most errors occur in the differential diagnosis of thyroid nodules. Dignity of cold nodules is too often explored histopathologically, diagnosis of indifferent nodules is made in too many cases and warm or hot nodules by no means do always represent focal autonomy. Great uncertainty persists in the evaluation of the functional relevance of autonomy.
