Abstract

Zusammenfassung

Eine steigende Lebenserwartung und Veränderungen im Lebensstil, die durch geringere physische Aktivität und eine höhere Aufnahme hochkalorischer Nahrungsmittel geprägt sind, tragen zu der weltweit steigenden Inzidenz von Diabetes mellitus bei. Neben den mikro- und makrovaskulären Komplikationen der Erkrankung sind Veränderungen am Skelettsystem Bestandteil der Komorbidität der Patienten. Besondere klinische Bedeutung haben Arthropathien und Periarthropathien an den Händen, Schultern und Füßen. Obwohl diese Veränderungen mit der Dauer des Diabetes mellitus und mit dem Alter der Patienten zunehmen, besteht eine direkte Assoziation zur Stoffwechseleinstellung nicht für alle Veränderungen. Auch eine Osteoarthritis tritt häufiger bei Patienten mit Diabetes mellitus auf. Die meisten skelettalen Komplikationen lassen sich anhand des klinischen Bildes diagnostizieren. Radiologische Befunde können ergänzend zur Differenzialdiagnose notwendig sein. Die Therapie der einzelnen Krankheitsbilder unterscheidet sich nicht von der Behandlung bei stoffwechselgesunden Patienten.

Summary

Longer life expectancy and modified lifestyle characterized by low physical activity and high-energy food intake contribute to a worldwide increasing incidence of diabetes mellitus. Skeletal abnormalities contribute in addition to micro- and macrovascular complications, to a significant comorbidity of the disease. Arthropathies and periarthropathies of hand, shoulder and foot are of special clinical interest. Osteoarthritis is more frequent in diabetes. The development of these disorders is dependent on age and on the duration of diabetes, however, it has been difficult to show a direct correlation with metabolic control of diabetes. Most of these skeletal abnormalities can be diagnosed clinically, but some radiological examinations may be helpful, especially in differential diagnosis. Treatment of arthropathies and periarthropathies in diabetic patients is not different from those used in general population.
