Zusammenfassung
Die Wirbelkörperfraktur (WKF) stellt eine Herausforderung für das öffentliche Gesundheitswesen dar. Die Interventionsmethoden an der Wirbelsäule wie Vertebroplastie und Kyphoplastie sind interessante, komplementäre Beiträge zur Behandlung der frakturierenden Osteoporose. Die Erarbeitung eines Konsensus der hauptsächlich Beteiligten an diesen Techniken ist notwendig geworden. Der Vorschlag eines solchen Konsensus der verschiedenen an der Betreuung von Patienten mit WKF Beteiligten ist unumgänglich. Im Hinblick auf die bereits vorliegende Literatur halten wir die Indikation für Vertebroplastie oder Kyphoplastie bei allen Patienten mit schmerzhafter WKF aufrecht. Falls der Patient bereits vor der WKF eine Kyphose aufwies oder falls die aktuelle WKF eine wesentliche lokale Kyphose bewirkt, ist eine Kyphoplastie indiziert. Falls der Patient eine bereits ältere WKF aufweist, deren konservative Behandlung jedoch zu keiner Schmerzlinderung führte, ist eine Vertebroplastie indiziert. In jedem Fall wird eine systematische Bilanz und eine medikamentöse Therapie der Osteoporose vorgeschlagen.
Summary
Vertebral osteoporotic fracture (VOF) is a major problem of public health. Surgical treatments such as vertebroplasty and kyphoplasty are interesting adjuvant treatments for the management of osteoporosis. A consensus proposed by the principal contributors of this management is important. Regarding the actual data, we propose a vertebroplasty or a kyphoplasty for all patients suffering of an acute VOF. If a previous kyphosis or an important local kyphosis does exist, secondary to the acute VOF or others, we propose a kyphoplasty. If the VOF is older and the conservative treatment is inefficient, we propose a vertebroplasty. In all cases, a specific management and treatment of the osteoporosis is proposed.
