Zusammenfassung
Varizenoperationen werden heute in der Mehrzahl unter ambulanten Bedingungen durchgeführt. Musste früher die Indikation zur ambulanten Operation besonders begründet wer-den, so gilt dies heutzutage für die stationäre Versorgung bei operativen Eingriffen am epifascialen Venensystem. Die technischen Standards und Anforderungen sind für die ambulante und die stationäre Versorgung gleich zu bewerten. Für die Frage der Indikationsstellung zur ambulanten bzw. stationären Durchfüh-rung sind neben Art und Umfang der Operation (technische Machbarkeit) die individuellen Be-dingungen seitens des Patienten zu bewerten (Zumutbarkeit). In der Bundesrepublik werden pro Jahr ca. 350 000 Operationen am oberflächlichen Venensystem durchgeführt davon nur noch ca. 120 000 unter stationären Bedingungen. Unter wirtschaftlichen Aspekten sind im gegenwärtigen Honorierungssystem der GKV die ambulante Varizenoperation, wie auch viele andere ambulante chirurgische Eingriffe, erheblich unterbezahlt.

Nowadays, most varicose vein operations are carried out under outpatient conditions. Whereas in the past, the indication for outpatient surgery had to be specially justified, today it is inpatient treatment for surgical procedures on the superficial venous system for which particular reasons are demanded. The technical standards and requirements for outpatient and inpatient treatment can be regarded as equal. When considering the indication for outpatient or inpatient surgery, in addition to the nature and extent of the operation (technical feasibility), it is important to assess conditions in the individual patient (appropriateness). Approx. 350,000 operations on the superficial venous system are carried out annually in Germany, of which only some 120,000 are under inpatient conditions. From an economic perspective, in the current remuneration system of the Statutory Health Insurers, outpatient varicose vein surgery is considerably underpaid – in common with many other outpatient surgical procedures.
