Abstract

Les valuers normales, la sensitivité, la spécificité et la précision diagnostique ainsi que le chevauchement des épreuves suivantes de la fonction thyroïdienne ont été définis par une étude prospective dans 300 cas. La captation thyroïdienne de l’¹³¹I à 2, 4, 10 et 24 h, le PB¹³¹I à 48 h, la captation de la T₃ marquée sur éponge de résine et la RUR (Resin-Uptake Ratio), la thyroxine sérique (Tetrasorb), le PBI (Protein-Bound Iodine) du sérum, les indices de la thyroxine libre (FTI = Free Thyroxine Index) calculés comme FTI₁ = PBI · RUR et FTI₂= thyroxine · RUR et le photomotogramme du réflexe achilléen ont été étudiés. Le chevauchement a été considérable pour toutes les épreuves effectuées. Le PB¹³¹I a été 1’ épreuve la plus fidèle pour l’hyperthyroïdie, et le FTI₂ pour l’hypothyroïdie. Ce dernier est considéré comme le procédé de choix pour l’examen préliminaire de tous les sujets contrôlés. Les malades hyper- ou hypo-thyroïdiens, avec un syndrome clinique franchement manifeste présentaient, en général, des valeurs plus anormales pour les diverses épreuves que les malades présentant un tableau clinique plus modéré. De même les sujets euthyroïdiens sans signes cliniques de maladie thyroïdienne présentaient au cours des divers épreuves des valeurs qui étaient plus franchement normales par rapport aux malades euthyroïdiens ayant des signes clinique suspects d’une maladie thyroïdienne, ce qui rendait le diagnostic du laboratoire plus facile. Les indications pour chaque épreuve thyroïdienne et l’interprétation des résultats sont discutés.

Summary

The normal range, the diagnostic sensitivity, specificity, accuracy and overlap have been defined in a prospective study of 300 cases for the following tests of thyroid function: 2, 4, 10 and 24 h ¹³¹I uptake, the PB¹³¹I at 48 h, the resin sponge uptake of labeled T₃ in vitro (Triosorb) and the resin uptake ratio (RUR), the serum thyroxine (Tetrasorb), the serum protein-bound iodine (PBI), the free thyroxine indices calculated as FTI₁ = PBI · RUR and FTI₂ - thyroxine · RUR, and the photomotogram of the Achilles reflex. There was a considerable overlap in all the tests studied. The PB₁₃₁I gave the best results in hyperthyroidism, and the FTI₂ in hypothyroidism. The latter is considered as the procedure of choice for overall screening purposes. Hyperthyroid and hypothyroid patients with a definite clinical syndrome had generally values more abnormal than patients in whom the clinical picture was less severe or less typical. Similarly, normal persons could be more easily differentiated with these tests from the hyper- and hypothyroid patients than euthyroid persons in whtfm abnormal thyroid function could not be definitely excluded by the clinical examination alone. The indications for the performance of these tests and their interpretation are discussed.

In 300 Fällen (Normalpersonen und verschiedene Schilddrüsenerkrankungen) wurde in einer prospektiven Studie der normale Wertbereich, die diagnostische Sensibilität, Spezifität, Genauigkeit und Überdeckung der Ergebnisse folgender Untersuchungen definiert: die Schilddrüsen-¹³¹ J-Aufnahme nach 2, 4, 10 und 24 Stunden, das proteingebundene Radiojod (PB¹³¹J) nach 48 Stunden, der T₃- in-vitro-Test (Triosorb), die Serum-Resin-Aufnahme Ratio (RUR), das Serum- Thyroxin (Tetrasorb), der freie Thyroxin-Index (1 und 2) FTI₁ = PBJ · RUR und FTI₂ = Thyroxin · RUR und das Photomotogramm des Achillessehnenreflexes. Eine deutliche Überdeckung der Ergebnisse der genannten Untersuchungen war zu verzeichnen. Das proteingebundene Radiojod (PB¹³¹J) ergab die besten Ergebnisse bei der Hyperthyreose und der FTI₂ bei der Hypothyreose. Der letzte gilt im allgemeinen als Test der Wahl bei der ersten Untersuchung. Bei Hyperthyreoseund Hypothyreose-Patienten, mit gut definiertem klinischem Syndrom, sind im allgemeinen die Werte viel ausgeprägter als bei Patienten mit weniger typischem bzw. ausgeprägtem klinischem Bild. Normale Personen können gleichfalls leichter mit diesen Tests von Hyper- und Hypothyreose-Patienten differenziert werden, im Gegensatz zu euthyreoten Personen, bei welchen durch die klinische Untersuchung allein eine abnormale Schilddrüsenfunktion nicht sicher auszuschließen ist. Die Indikationen für die Anwendung dieser Tests und ihre Interpretation werden diskutiert.
