The DOTATOC PET-CT as a potential new method for diagnostic imaging of pleomorphic adenomas

    Introduction The surgical resection of pleomorphic adenomas is recommended due to the risk of malignant transformation. Current preoperative diagnostic imaging to determine the dignity of a suspected pleomorphic adenoma is non-specific. Recent studies have described the expression of somatostatin in pleomorphic adenomas. Goal of this study is to determine if the DOTATOC PET-CT may be used to identify pleomorphic adenomas.

    Methods inclusion parameters for this monocentric retrospective observational study were all patients who received a DOTATOC PET-CT between 01.01.2010 and 12.31.2018 and who also received a saliva gland tumor resection (n = 14). Image assessment was performed through qualitative and semiquantitative analysis.

    Results Of the 14 identified patients 5 patients had histologically confirmed pleomorphic adenomas. DOTATOC tracer-uptake was found to be increased in 80 % of these cases. The smallest PA (1,5ml) tumor showed no tracer-uptake. By 3 patients could histologically be diagnosed a Warthin-tumor, additionally an oncocytoma, lymphoma, a squamous cell carcinoma lymphnode metastasis, a granuloma, and chronical parotitis. No other tumors showed a relevant tracer-uptake. The average PA tumor volume was 13,3 ml (with a range of 1,5ml-55ml).

    Conclusions This unknown diagnostic property with Tracer-Uptake in pleomorphic adenomas should be further researched, and could be utilized to preoperatively identify pleomorphic adenomas.
