Introduction Lipomas are the most common soft-tissue tumors in the body. A prevertebral occurance
is rare and often only lead to symptoms when they reach large size.
Methods A 52 year old patient presented with coughing and the constant urge to clear the
throat. The examination revealed a posterior pharyngeal wall bulging extending from
epipharnyx to hypopharnynx. An outpatient performed MRI presented a large homogeneous
mass originated in the prevertebral space. Total excision is the treatment of choice.
A transcervial surgical approach and a transoral excision were discussed.
Results We performed the transoral surgical excision under general anasthesia. The tumor
was completely removed. The histopathologic diagnosis of the lesion was lipoma. Following
the excision the patient was relieved from the complaints.
Conclusion With the possibility of increasing size the complete surgical removal is the first
choice of treatment. A transoral resection of a huge prevertebral lipoma can be performed.
