Horizontal canal BPPV is not so common as posterior canal BPPV, but it may cause more
severe symptoms and prolonged duration.
We had the last year in our office370 patients with BPPV. 90 of them suffered from
h-BPPV. 15 of them after were treated for p-BPPV. Most of them were women and were
diagnosed after the supine roll test. We used Gufoni, Vannnuchi-Asprella and Barbecue
roll test. In order to have more precise results we used the methods randomly, trying
to have the same number of patients for each method. After the repositioning maneuvers
patients were recommended to basic postural restrictions.
Overall success rates were 90 %, but the maneuvers were repeated until the patients
were free of symptoms. Recurrences occurred in 30 % of patients treated with Gufoni
and Vannnuchi-Asprella method and only 10 % of patients with Barbecue roll method.
All the methods are effective for the therapy of h-BPPV, whereas Barbecue roll method
remains the most effective as the vertigo resolves in less sessions than the other
methods and the rate of recurrence is smaller. It is therefore more difficult to applicate
it in obese patients.
