Zusammenfassung

Mit der Vierten Fortschreibung der Richtlinie der Bundesärztekammer zur Feststellung des irreversiblen Hirnfunktionsausfalls („Hirntod“), gültig seit Juli 2015, erfolgte in einigen Aspekten auch eine Präzisierung in Bezug auf die Erhebung der klinischen Symptome. Insbesondere sind darin die zu prüfenden Voraussetzungen ausführlicher dargestellt, und es wurde eine Untergrenze für den Ausgangs-Wert des arteriellen Kohlenstoffdioxid-Partialdruckes vor Durchführung des Apnoe-Tests festgelegt. Der vorliegende Artikel stellt auf aktuellem Kenntnisstand die pathophysiologischen Grundlagen sowie die praktische Durchführung der klinischen Befunderhebung des irreversiblen Hirnfunktionsausfalls, einschließlich des Apnoe-Tests, sowie das Vorgehen bei eingeschränkter Untersuchbarkeit in detaillierter Form dar. Zudem werden die beobachtbaren – und mit dem Hirnfunktionsausfall vereinbaren – spinal-motorischen Phänomene und die Kriterien für deren Abgrenzung von zerebralen Reaktionen beschrieben.

Abstract

With the fourth update of the German Medical Association’s guideline for the determination of irreversible loss of brain function (“brain death”), effective from July 2015, a more detailed specification of some aspects of the clinical investigation was included. In particular, the preconditions to be scrutinized are described more extensively, and the lower limit for the arterial partial pressure of carbon dioxide before carrying out the apnea test is defined. The present article delineates in detail the current state of knowledge, the pathophysiological basis and the practical procedure for ascertaining the clinical features of irreversible loss of brain function, including the apnea test, and the line of action in case of limited clinical assessability. In addition, the observable spinal reflexes and automatisms compatible with brain death as well as the criteria used for their discrimination from cerebral responses are described.
