Dissection Using the Pocket-Creation Method – Endoscopic Submucosal Dissection of Polypoid Lesion of the Sygmoid
Endoscopic submucosal dissection (ESD) is an effective technique for the complete resection of polypoid lesions.Male patient, 73 years old, colonoscopy revealed a sessile polyp of 40 mm in the sigmoid, JNET 2B.The patient was proposed for ESD. A solution of indigo carmine and adrenalin was injected into the submucosa, followed by a mucosal incision. Submucosal dissection was performed by pocket-creation method, with en bloc resection. Pathology evaluation revealed the lesion was an adenomatous polyp with low grade dysplasia and negative margins.This case demonstrates the advantage of the pocket-creation method, allowing the complete resection of a large lesion.
