Introduction Structured reporting (SR) has been shown to increase report completeness and time efficiency of head and neck ultrasound. Nevertheless, there is no data concerning the impact of SR on inter-rater reliability of head and neck ultrasound, which is often criticized to be inferior to other imaging techniques. Therefore, the aim of this study was to compare completeness and consequently inter-rater reliability of head and neck ultrasound.

Materials and Methods n=4 head and neck ultrasound examinations of typical pathologies were captured by video/images by 2 DEGUM instructors. In a next step, video/image files were sent out to n=8 ENT departments with DEGUM instructors on staff. SRs were created using a specific templated based upon the files provided (n=32 reports). Reports were assessed for overall completeness and inter-rater reliability. Additionally, user satisfaction was evaluated by visual analogue scale (VAS).

Results SR produced a very good mean overall report completeness (91.6 %) in all 4 cases (86.4 % - 95.6 %). Additionally, inter-rater reliability was substantial using SR with a Fleiss‘ kappa of 0.73 (0.64 - 0.94) and an inter-rater agreement of 87.2 % (50 % - 100 %). User satisfaction of the SR template was also very good (VAS 8.6).

Conclusion Implementation of SR of head and neck ultrasound leads to a very good comparability while assuring very high report completeness and user satisfaction ratings. Additionally, SR enables scientific data to be collected and evaluated with ease. Consequently, SR may lead to a sustained improvement in process quality and workflow, especially for follow-up examinations.
