Frequency and risk factors of MRSA colonization in ENT patients: results of a prospective surveillance cohort in an university ENT department

    Content Introduction Infection and colonization with Methicillin resistant staphylococcus aureus (MRSA) has become a growing problem to health care systems worldwide within the last decades leading to higher expenses and an increased morbidity and mortality. This prospective surveillance study aims to identify the frequency of and risk factors for MRSA colonization in ENT patients.

    Methods Data were prospectively collected from all patients being admitted for surgical or conservative therapy at the ENT department of the University Hospital Münster from October 2015 until September 2020. All positive findings were evaluated including whole genome sequencing to detect genotypic relationships of different MRSA strains. Based on a structured literature review, the following risk factors for MRSA colonialization in ENT patients were detected: age, hemato-oncological diseases, antibiotic therapy, tracheotomy and PEG. Using a retrospective chart review, Patients were classified based on positive findings and compared to an age-matched control group.

    Results 243 patients (60.9 %  male; 39.1 %  female) were found to be colonised with MRSA. Age ranged from 0.47 to 93.07 years with a mean age of 48.81 years. Most frequently MRSA was found in the nasopharyngeal space (69.5 % ). 25 patients (10.3 % ) presented with a previous history of MRSA. Possible risk factors were analysed for risk assessment and to compute a risk score. Hemato-oncological diseases were found in 24.3 % .

    Discussion/Conclusion MRSA Screening in ENT clinics could be established on a clinically based risk factor, if ressources are too scarce to perform MRSA screening in all patients. Persistence of inflammation is suggestive for MRSA, if one or more of the above mentioned risk factors are fulfilled.
