A new innovative method in trachea reconstruction

    Introduction The trachea reconstruction after tracheal malignancy resection is difficult and challenging. In this case report, a new innovative tracheal reconstruction with cervical bilobed flap is demonstrated.

    Material and method A 50-year-old patient who presented to our clinic with acute dyspnea. He was known with pT4, pN1, M0, L0, V0, R0 oral floor cancer and lower jaw infiltration. Tumor resection in October 2017 and postoperative radiation. An emergency tracheotomy and panendoscopy were performed. Intraoperatively, a tracheal tumor about 3 cm in size was found. This was biopsied and histologically it was found to be a squamous cell carcinoma G II. Staging was carried out. This showed a cT4a tracheal carcinoma with thyroid infiltration on the left. The case was presented at the tumor conference and was initially recommended for surgery if resection was possible.

    Results and course A surgical resection of pT4a, cN0, cM0, L1, V1, R0, G2 was performed in October 2019. A trachea resection and hemithyroidectomy on the left and central neck dissection level VI on both sides was done. The defect in the area of ??the trachea posterior wall and side wall on both sides was reconstructed with the help of cervical bilobed flaps on both sides. A postoperative radiation therapy was performed from mid-December 2019 to the end of January 2020. The clinical and sonographic examination showed no evidence of a relapse with a good general condition and good functional swallowing and breathing function one year postoperatively.

    Summary It can be shown that the reconstruction of the trachea with the help of cervical bilobed flap is a good, safe and low-risk option
