The Role of Fibrinogen-Thrombin Coated Collagen Sponge (TachoSil©) in Rhinoplasty: Our Experience with 78 patients

    Introduction In order to achieve a regular and harmonious bridge of the nose during rhinoplasty, various methods are described.

    Objectives We investigated the postoperative results when using a thrombin/fibrinogen collagen matrix (TachoSil®) partially in combination with cartilage grafts for augmentation of the nasal septum.

    Material and methods From 01.01.2016 - 29.07.2019 TachoSil® was used in 78 patients undergoing rhinoplasty. We retrospectively analyzed the clinical data and postoperative follow-up up to 6 months after surgery including patient satisfaction.

    The results In all cases the surgery was performed in closed technique. The gender distribution was 48/30, the average age was 31 years. Forty-eight patients were female and thirty patients were male with a mean age of 31 years. Initial surgery was performed in 53 patients and revision surgery in 25 patients.

    There were no intolerance reactions to TachoSil® or postoperative bleeding. In all cases an optically smooth contour of the bridge of the nose was observed, no skin discoloration was observed. 85 %  of the patients were satisfied, while 15 %  were not yet satisfied with the result. Further details were mentioned in the results section. Two patients presented on the 10th postoperative day with an abscess in the bridge of the nose.

    Conclusion The use of a thrombrin/fibrionogen matrix (Tachosil) represents an effective, safe, and easy-to-use graft fixation technique to conceal irregularities of the bridge of the nose and to augment the soft tissue in thin-skinned patients.
