En bloc excision for advanced laterally spreading colonic lesions has many advantages, including reduced recurrence, improved pathological assessment, and potential cure in low risk submucosal invasive cancer. The currently available platforms for colonic endoscopic submucosal dissection have significant clinical limitations. The authors present the favorable results of an in vivo animal model experience with a novel device for colonic submucosal dissection, that incorporates bipolar radiofrequency for cutting, microwave coagulation for hemostasis, and a retractable needle for injection. Human trials are awaited.
