Hyperparathyroidism and Multiple Pathological Fracture Caused by Parathyroid Carcinoma: A Case Report

Introduction: Parathyroid carcinoma is a rare cause of primary hyperparathyroidism. It is often presents with unspecified clinical manifestation. We are here report a case of a 27-year-old man who suffered multiple recurrent bone fracture for 5 years. Now the patient complained of lower limbs weakness and unable to walking, urinating blood, pain in the waist that radiates to the lower abdomen.

Methods: Examination revealed a neck mass, mimicking a goiter. X-ray of right humerus and bilateral femur showed nonhealing fracture.

Results: Biochemical findings found an elevated parathyroid hormone level and hypercalcemia. Sestamibi Imaging showed us a right inferior parathyroid mass that infiltrating the right lobe of thyroid. The patient underwent right isthmolobectomy and parathyroidectomy. Histopathology results confirmed a parathyroid carcinoma. He achieved normalization of serum calcium and parathyroid hormone levels in 2-months postoperation.

Conclusion: The diagnosis of parathyroid carcinoma continues to be a challenge. Understanding the pathogenesis and multidisciplinary collaboration is important to define an accurate diagnosis and treatment.
