Zusammenfassung
Fragestellung: Das TNM-System zur Klassifikation der malignen Tumoren wurde 1997 in seiner 5. Auflage veröffentlicht. Die wichtigste Änderung zur 4. Auflage wurde bei der Beurteilung des T-Stadiums vorgenommen. In der vorliegenden Arbeit wurde die prognostische Vorhersagekraft des T-Stadiums beider Auflagen verglichen.
Material und Methoden: 1975 bis 1997 wurden 2014 Patienten wegen eines Nierenzellkarzinoms operativ behandelt. In einer retrospektiven Analyse konnten 1794 Patientendaten berücksichtigt werden. Die Überlebenszeitanalysen wurden nach Kaplan-Meier berechnet.
Ergebnisse: Hinsichtlich der prognostischen Vorhersagekraft der einzelnen T-Stadien zeigte die 5. Auflage einen deutlichen, signifikanten Überlebensvorteil der Patienten im T1-Tumorstadium, der bei der 4. Auflage nicht festzustellen war. Wird der „cut off” zwischen T1 und T2 anstatt bei 7 cm bei 4 cm gelegt, bleibt die prognostische Wertigkeit weiterhin bestehen.
Schlussfolgerung: Die 5. Auflage der TNM-Klassifikation bei Nierentumoren weist in der Beurteilung der Prognose einen Vorteil gegenüber der 4. Auflage auf. Eine Änderung des T-Stadiums mit einem „cut off” von 4 cm zwischen T1 und T2 würde in Kliniken, in denen auch bei elektiver Indikation organerhaltend operiert wird, hilfreich bei der Therapieentscheidung sein. T1-Tumoren könnten organerhaltend, T2-Tumoren dürften nicht mehr organerhaltend operiert werden. Somit würde eine solche alternative Einteilung einen Vorteil gegenüber der bestehenden Klassifikation darstellen.
Abstract
Introduction: In 1997, the 5th edition of the TNM classification of malignancies was published. The most significant change from the 4th edition was made in the T-stage. We compared both TNM staging classifications in terms of accuracy in predicting patient outcome.
Materials and Methods: From 1975 to 1997, a total of 2014 patients underwent surgical treatment for renal cell carcinoma at our hospital. The data of 1794 patients could be used in this retrospective analysis. Kaplan-Meier survival curves were generated and compared.
Results: With regard to the prediction of patient outcome, the 5th edition of the TNM staging classification - with a tumor size “cut off” of 7.0 cm between stages T1 and T2 - shows a significantly higher survival rate for stage T1 patients than the previous edition (“cut off” 2.5 cm). In addition, the same prognostic accuracy as the 5th edition was achieved with a “cut off” of 4 cm.
Conclusion: The 5th edition of the TNM classification shows a significant advantage in prognosis over the 4 th edition. An alternative classification (T1 < 4 cm, T2 ≥ 4 cm), however, may furthermore improve clinical usefulness in decision-making for organsparing surgery in elective indications. In this instance, stage T1 patients could be selected for partial nephrectomy, while stage T2 patients could not. Such an alternative classification permits better patient selection and is thus clinically more useful than the current TNM staging classification.
