Praxis der Thromboseprophylaxe in der Neurochirurgie: Ergebnisse einer Umfrage in Deutschland

Thromboembolische Komplikationen stellen eine häufige Ursache perioperativer Morbidität und Mortalität nach neurochirurgischen Eingriffen dar. Aufgrund der potentiell schwerwiegenden Auswirkungen einer intrakraniellen oder spinalen Nachblutung und der teilweise aktuell publizierten kontroversen Befunde in der Literatur fehlt ein einheitliches Konzept zur Thromboseprophylaxe in der Neurochirurgie. Das Ziel der vorliegenden Arbeit bestand darin, einen Überblick über die Praxis der medikamentösen Thromboseprophylaxe in Deutschland zu erhalten und den Stand der Literatur zu analysieren, um eine Orientierung für die klinische Anwendung zu geben.

In einer postalischen Umfrage wurde aus 34 deutschen neurochirurgischen Zentren Informationen über die Art und den Zeitpunkt des Beginns der medikamentösen Thromboseprophylaxe in den einzelnen Zentren gesammelt. Alle Angaben bezogen sich auf einen präoperativ wachen, mobilen, elektiven neurochirurgischen Patienten. Je nach operativem Eingriff wird in 88% bis 97% der befragten Einrichtungen eine medikamentöse Thromboseprophylaxe durchgeführt. Bei Kraniotomien wurde in 2 Kliniken (6%) routinemäßig präoperativ, in 10 Kliniken (29%) postoperativ innerhalb der ersten 8 Stunden, in 15 Kliniken (44%) postoperativ innerhalb der ersten 24 Stunden, in einer Klinik (3%) am zweiten postoperativen Tag und in 5 Kliniken (15%) am dritten postoperativen Tag begonnen. Die Zahlen für Bohrlochtrepanationen und spinale Eingriffe unterscheiden sich davon nur geringfügig. Die Ergebnisse dieser Umfrage zeigen im Vergleich zu anderen Ländern einen häufigeren Einsatz von low-dose Heparin oder niedermolekularem Heparin.

Summary:

Many neurosurgical patients are at high risk of developing thromboembolic disease. Although recent data support the benefit of pharmacological thromboembolic prophylaxis, the optimal starting point for the different neurosurgical procedures remains unclear because the potentially devastating consequences of intracranial or intraspinal haemorrhage could easily offset the reduction in thromboembolic morbidity and mortality resulting from pharmacological thromboembolic prophylaxis. The objective of our study was to assess the use of pharmacological methods of thromboembolic prophylaxis in contemporary German neurosurgery using a postal survey in 34 neurosurgical departments.

Low-dose heparin or low molecular weight heparin (LMWH) were used in 88% to 97% of neurosurgical units participating in our survey. In craniotomies, in 2 units (6%) heparin or LMWH was routinely started before surgery, in 10 units (29%) < 8 hours after surgery, in 15 units (44%) < 24 hours after surgery, in one unit at the second day and in 5 units at the third day after surgery. Data for burr hole craniotomy or spinal surgery were similar. In our survey a high rate of perioperative use of pharmacological thromboembolic prophylaxis was found. However, given the data from the literature, the optimum protocol remains unclear.
