Ruptur einer Echinokokkuszyste in die Gallewege

Zusammenfassung.

Durch eine veränderte Bevölkerungsstruktur mit höherem Anteil von Patienten aus Endemiegebieten und vermehrten Urlaubsreisen in mediterrane Länder muss die Echinokokkose in die Differenzialdiagnose der rechtsseitigen Oberbauchbeschwerden und der fokalen Leberläsionen miteinbezogen werden. Dabei kommt dem Ultraschall eine besondere Bedeutung zu. Zusammen mit serologischen Untersuchungen liefert die Computertomographie in nahezu 100 % der Fälle die Diagnose. Trotz verbesserter konservativer Behandlung und interventioneller Techniken bleibt die Operation die Therapie der Wahl. Dies gilt auch für den Einbruch der Parasiten ins Gallengangsystem, obwohl in letzter Zeit rein endoskopisch behandelte Fälle publiziert wurden. Solange keine Langzeitergebnisse vorliegen, gilt die Laparotomie mit Zystektomie und Choledochotomie mit T-Drainagen-Behandlung in Kombination mit einer begleitenden medikamentösen Therapie als Standardtherapie bei Hydatideneinbruch in das Gallengangssystem. Bei der operativen Behandlung von Echinokokkuszysten sollten die Voraussetzungen für eine Leberteilresektion vorhanden sein.

Intrabiliary rupture of hydatoid cyst - a case report.

Changes of population structure with a higher proportion of patients from endemic areas and the increasing number of vacation periods in mediterranean countries require attention to echinococcus infection in the differential diagnosis of right-sided abdominal pain and focal liver lesions. Abdominal ultrasound is the most important preliminary technical examinations. Together with serological examinations computed tomography confirmes the diagnosis in nearly 100 %. Surgical intervention is the standard treatment. This is especially true in cases where the biliary system is affected although endoscopic techniques in treatment of the disease have been published. Until long-term results will be available the open surgical technique with cystectomy and choledochotomy with T-tube treatment in combination with medical treatment is still the established approach. The surgical treatment of this special complication of echinoccus cysts should be limited to hospitals with adequate experience in liver resection.
