Notfälle bei Vaskulitiden[1]

Zusammenfassung

Vaskulitiden verursachen die meisten Komplikationen durch Ischämien. Das klinische Bild ist dabei abhängig von der Größe der betroffenen Gefäße. Bei der Takayasu-Arteriitis sind die Aorta und ihre Äste betroffen mit der Folge von Apoplexie, Ischämie der Extremitäten, renovaskulärer arterieller Hypertonie, aber auch pulmonaler Hypertonie. 50 mg/d Prednisolon, Angioplastie und Bypass-Operation sind als gesicherte Therapien anzusehen. Bei 71 konsekutiven Patienten mit akuter Arteriitis temporalis aus Berlin-Buch traten zu 39 % Sehstörungen auf (14 Patienten mit akuter anteriorer ischämischer Optikusneuropathie, 8 mit Amaurosis fugax, je 2 mit Augenmuskelparesen und Cotton-Wool Exsudaten, je 1 mit Zentralarterien- und Astverschluss). Je 2 Patienten hatten einen Myokardinfarkt, Apoplex, thorakales Aortenaneurysma, A. brachialis/axillaris Stenose. Bei klinischem Verdacht auf Arteriitis temporalis sollte sofort mit 70 mg/d, bei Komplikationen mit 200 - 1000 mg/d Prednisolon behandelt werden. Das Kawasaki-Syndrom ist eine akute Vaskulitis meist im Kindesalter, die hochdosiert mit Acetylsalicylsäure und Immunglobulinen behandelt wird. Die cPAN wird häufig durch Mesenterialinfarkt und ischämisches Nierenversagen kompliziert. Vaskulitiden der kleinen Gefäße (M. Wegener, mPAN, Churg-Strauss Syndrom, Purpura Schönlein-Henoch und essenzielle Kryoglobulinämie) sind durch Alveolitis, Glomerulonephritis, intestinale Vaskulitis und Neuritis kompliziert. Frühzeitige Therapie mit 2 - 15 mg/kg Prednisolon und 2 mg/kg Cyclophosphamid ist erforderlich.

Emergencies in Vasculitides

Most complications in vasculitides are caused by ischemia. The clinical image depends on the size of the vessels involved. In Takayasu’s arteritis the aorta and its branches are inflamed. Thus strokes, limb ischemia, renovascular arterial hypertension and mesenteric ischemia occur as well as pulmonary hypertension due to vasculitis of the pulmonary arteries and myocardial infarction due to involvement of the ascending aorta. Established therapies in acute complications of Takayasu’s arteriitis are 50 mg/d prednisolone, angioplasty, and bypass-surgery. In 71 consecutive patients with acute temporal arteritis we found 39 % with ophthalmological complications such as anterior ischemic optic neuropathy (14 patients), amaurosis fugax (8 patients), paralysis of ocular muscles (2 patients), and cotton-wool exudates (2 patients). One patient had an occlusion of the central retinal artery, another one an occlusion of a ramus. Two of 71 patients with acute temporal arteritis had myocardial infarction, stroke, thoracal aortic aneurysm, and occlusion of the brachial/axillary artery, respectively. If temporal arteritis is suspected, corticosteroid therapy should be started immediately. A daily dose of 70 mg for patients without complications and 200 to 1000 mg for patients with complications is recommended. Kawasaki’s syndrome is an acute vasculitis occurring predominantly in children. It is treated with high doses of acetylsalicylic acid and a single high dose of immunoglobulins. Small-vessel vasculitides (Wegener’s granulomatosis, mPAN, Churg-Strauss syndrome, Henoch-Schönlein purpura, and mixed cryoglobulinemia) are complicated by alveolitis, glomerulonephritis, intestinal ischemia, and neuropathy. Early treatment with 2 to 15 mg/kg/d prednisolone combined with 2 mg/kg/d cyclosporine is essentially required.
