Manifestation eines primären malignen Melanoms in einem reifen Teratom des Ovars

Zusammenfassung

In reifen Teratomen des Ovars kommen maligne Veränderungen nur in 0,2 % bis 1,8 % der Fälle vor. Primär maligne Melanome des Ovars sind wirkliche Raritäten. Bei Anwendung strenger Kriterien zur Diagnose wurden in der Literatur weltweit bisher nur 15 solcher Fälle beschrieben. Die vorliegende Kasuistik berichtet über eine 70-jährige Patientin, die an einem primären malignen Melanom in einem reifen Teratom des Ovars erkrankte. Zum Operationszeitpunkt fanden sich auch zwei Lebermetastasen. Neben einer Hysterektomie und Adnexektomie beidseits erfolgte eine Entfernung der Lebermetastasen durch eine Lobektomie der Segmente III und IV. Eine adjuvante Chemotherapie mit Vincristin, Dacarbazin, Cyclophosphamid und Doxorubicin schloss sich an. Die Patientin verstarb 16 Monate nach der Primärdiagnose an der Tumorprogression und Kachexie. Die Literatur weist darauf hin, dass neben einer primären operativen Therapie eine kombinierte adjuvante Chemo- und Immuntherapie zur Verbesserung der ingesamt ungünstigen Prognose von klinischem Nutzen sein könnte. Eine platinhaltige Chemotherapie scheint neben dem klassischen Dacarbazin am ehesten wirksam zu sein. 5-Jahres-Überlebensraten zwischen 4 und 60 Monaten wurden berichtet.

Summary

Malignant changes are seen in 0.2 % to 1.8 % of mature teratomas of the ovary and only 15 cases of primary malignant melanoma of the ovary, with survival times between 4 and 60 months, have been reported in the world literature. We describe a 70-year-old patient who was found to have a primary malignant melanoma associated with a mature teratoma and two liver metastases. Total abdominal hysterectomy with bilateral salpingo-oophorectomy and resection of the hepatic segments III and IV were performed. The patient received adjuvant chemotherapy with vincristine, dacarbazine, cyclophosphamide and doxorubicin but died of disease at 16 months. The literature suggests that surgery followed by platinum-based chemotherapy, dacarbazine and immunotherapy may improve the unfavorable prognosis of patients with this condition.
