Myokymien des Gesichtes und Spasmus hemifazialis

Zusammenfassung

Im Bereich der mimischen Muskulatur lassen sich zwei Formen unwillkürlicher Aktivität beobachten: Myokymie und hemifazialer Spasmus. Die Myokymie geht klinisch mit als Muskelwogen beschriebener, kontinuierlicher, nichtsynchronisierter Bewegung einher, die sich elektromyographisch als kurze Entladungsserien einzelner oder mehrerer motorischer Einheiten darstellt, welche mehr oder weniger rhythmisch mit einer Frequenz von 1 - 5 Hz wiederkehren. Auch die Entladungsfrequenz und das Interspikeintervall der motorischen Einheiten innerhalb dieser Salven können hierbei variieren. Faziale Myokymien sind meist Folge von entzündlichen oder raumfordernden Prozessen im Hirnstamm. Daneben treten sie auch generalisiert als eines von mehreren Phänomenen im Rahmen einer Hyperexzitabilität peripherer Neurone wie etwa bei Polyradikulitiden, nach Schlangengiftintoxikationen oder bei der Neuromyotonie auf. Während der exakte Pathomechanismus der umschriebenen Formen noch unklar ist, lässt sich für viele generalisierte Formen eine Funktionsstörung des spannungsabhängigen Kaliumkanals nachweisen, der bei der Neuromyotonie vermutlich immunologisch, bei der episodischen Ataxie durch eine Mutation des KCNA1-Gens bedingt ist. Der Spasmus hemifazialis ist demgegenüber eine durch Willkürbewegung induzierte, aber auch spontan auftretende repetitive Impulsgenerierung in demyelinisierten Nervenfasern mit ephaptischer Erregungsausbreitung auf benachbarte Axone, die zu synchronen Zuckungen der mimischen Muskulatur einer Gesichtshälfte führt. Ursache ist zumeist eine Kompression durch eine Gefäßschlinge im Wurzelaustrittsbereich.

Abstract

Two types of involuntary movement's can be observed in the facial musculature: myokymia and hemifacial spasm. Myokymia is a condition involving undulating, rippling movements of muscle fibres, associated with the electromyographic finding of brief bursts of individual motor unit potentials recurring semirhythmically several times per second. Myokymia occurs in the face with intrinsic brainstem lesions in multiple sclerosis or in brainstem tumours. As a general condition, it is frequently caused by a Guillain-Barré syndrome, by rattlesnake venom intoxication or is seen in neuromyotonia. The exact mechanism of localised myokymia is still unknown. Functional deafferentiation, hyperexcitability of the facial nucleus but also focal demyelination are implicated as possible causes. In generalised forms, abnormalities mainly of the voltage-gated K⁺ Channel result in hyperexcitability of the peripheral nerve. The hemifacial spasm also results from focal hyperexcitability of the VII^th nerve in most cases caused by vascular compression. It is, however, characterised by simultaneous contractions of the facial muscles due to ephaptic impulses triggered by voluntary activity or spontaneously. Electromyographic characteristics allow differentiation from myokymia or synkinesias.
