Zusammenfassung
Fragestellung: Die vorliegende Untersuchung hat zwei Ziele: Einmal soll ein Überblick darüber gegeben werden, welche Methoden bei Rehabilitationsmaßnahmen an der See bei Neurodermitispatienten angewendet werden. Zweitens soll der Erfolg der verschiedenen therapeutischen Maßnahmen quantifiziert werden. Methode: In vier stationären Einrichtungen an der Nord- und Ostsee nahmen 667 Patienten zwischen 3 und 65 Jahren mit der Diagnose Neurodermitis an der Untersuchung teil. Der dermatologische Befund wurde anhand des EASI-Scores objektiviert. Die Ärzte wurden gebeten, das in ihren Kliniken übliche Behandlungsregime anzuwenden. Ergebnisse: Während der stationären Heilmaßnahme sank der durchschnittliche EASI-Score von 28 auf 13. Von den 667 Patienten zeigten 621 (93 %) eine Verbesserung des Hautbefundes. 11 Patienten (2 %) zeigten keine Veränderung in der Schwere ihrer Symptomatik, 35 Patienten (5 %) eine Verschlechterung. Bei fast allen Patienten wurde eine Basistherapie verordnet, die aus Meerwasserbädern und künstlicher UV-Bestrahlung besteht, die die natürliche Sonnenstrahlung ergänzen soll. Therapeutika wie Farbstoffe, LCD oder Steroide wurden in den einzelnen Kliniken nach unterschiedlichen Strategien verordnet, wobei in der Regel eine Korrelation zum Ausgangsbefund deutlich wurde. Der therapeutische Effekt war weitgehend davon unabhängig, welche Therapeutika eingesetzt worden sind. Schlussfolgerungen: Die Ergebnisse der Untersuchung haben die therapeutische Wirksamkeit eines stationären Aufenthaltes an der See überzeugend nachweisen können. Die Frage, welchen Anteil am therapeutischen Erfolg die thalassotherapeutischen Maßnahmen und welchen Anteil die spezifischen dermatologischen Therapeutika haben, musste jedoch unbeantwortet bleiben, u. a. deswegen, weil die Therapien sehr gezielt und abgestuft nach der Schwere des jeweiligen Befundes eingesetzt wurden.
Abstract
Purpose: The purpose of this study is twofold: first to give a survey on rehabilitation measures that are used at the seaside for patients suffering from atopic dermatitis. Secondly, it aims to quantify the effect of the different therapeutic measures. Method: 667 patients aged between 3 and 65 years were treated for atopic dermatitis in 4 rehabilitation hospitals on the cost of the North Sea and Baltic Sea. The extent and severity of the disease were quantified using the EASI- Score (Eczema Area and Severity Index). The patients were treated according to the regimen normally used in the respective hospital. Results: After four to six weeks of treatment, the average EASI-Score dropped from 28 to 13. 621 (93 %) of the 667 patients improved. 11 patients (2 %) were discharged with equal and 35 patients (5 %) with higher EASI-Scores. All patients received a baseline therapy consisting of seawater and artificial UV-radiation, which is meant to supplement solar UV-radiation. Therapeutic agents, such as tar, LCD or steroids, which are more likely to cause side-effects, were administered according to the regimens usually followed in the different hospitals. A correlation between the EASI-Score at admission and the frequency of administration of more potent drugs became obvious. The therapeutic effect of the cure was not correlated with the administered drugs. This is probably due to the fact that more efficacious drugs were used when therapeutic failure was impending. Conclusions: The results of the study provide convincing evidence for the therapeutic benefit of rehabilitation measures at the seaside for patients suffering from atopic dermatitis. Since a whole set of therapeutic measures was administered according to the patient's individual response, it could not be differentiated, however, to which extent each different measure contributed to the outcome.
