Gesundheitsökonomische Aspekte des Kolonkarzinoms: Prävention/Behandlung oder Nachsorge - welches ist der günstigste Ansatz ?

In Deutschland versterben pro Jahr fast 30 000 Menschen an den Folgen eines kolorektalen Karzinoms. Dies hat auch enorme gesamtwirtschaftliche Folgen. Für die Behandlung des Tumors müssen jährlich über 600 Millionen € aufgewendet werden, ungeachtet der hinzukommenden indirekten und intangiblen Kosten. Der Krebs ist jedoch durch Anwendung präventiver Maßnahmen zu 75 - 90 % vermeidbar. Der ökonomisch günstigste Ansatz ist daher die Vermeidung des Tumors. Maßnahmen zur primären Prävention sind kostengünstig, einfach durchführbar und sehr effektiv. Da diese Maßnahmen nur von einem geringen Teil der Bevölkerung wahrgenommen werden, kommt den Früherkennungsmaßnahmen eine um so größere Bedeutung zu. Methode der Wahl ist derzeit der kostengünstige jährliche Test auf okkultes Blut im Stuhl ab dem 50. Lebensjahr in Kombination mit einer Sigmoidoskopie alle 5 Jahre. Die Koloskopie als primäre Screeningmaßnahme wird zur Zeit (noch) nicht empfohlen. Alle diese genannten Screeningstrategien liegen unter der Kosteneffektivitätsgrenze von 50.000 US-$ pro gerettetem Lebensjahr und gelten damit im Vergleich zu anderen Tumorentitäten als sehr kosteneffektiv. Die Sensibilität und Akzeptanz zur Früherkennung in der breiten Bevölkerung ist jedoch noch deutlich verbesserungswürdig. Der vorliegende Beitrag gibt einen Überblick über Effizienz und Kosteneffektivität der verschiedenen Screeningmethoden zur Prävention des Kolonkarzinoms.

Abstract

Colorectal cancer is the second leading cause of cancer-related deaths in Germany and has an enormous economic impact on the society. Colorectal cancer incidence rate can be reduced by 50 % through primary prevention, which is effective and economical. However, most people are not aware of the powerful tools of primary prevention and hence it is vital to propose screening strategies for detecting adenomatous polyps or the tumour at an early stage. Originally, the recommended secondary prevention strategy contains annual fecal-occult blood testing in combination with sigmoidoscopy once every 5 years. More recently, screening with colonoscopy has become the preferred choice of many experts and is now also recommended in the guidelines. All cost-effective ratios of these two recommended strategies are under the threshold of $ 50.000 per live year saved and are comparable with other cost-effective cancer screening strategies. This paper gives an overview of the efficacy and cost-effectiveness of the screening strategies in colorectal cancer.
