Zusammenfassung
Ein 63 Jahre alter Mann klagte über beidseitige Klaudikationsbeschwerden. Mittels abdomineller Angiographie wurde ein Verschluss der unteren Bauchaorta diagnostiziert, der in beide Iliakalarterien hineinreichte. Es wurde eine Bifurkationsprothese implantiert und bei der Operation eine Probe des thrombotischen Verschlussmaterials zur histologischen Untersuchung entnommen. Diese erbrachte den Nachweis von atypischen Zellen im Thrombus. Daraufhin wurde die zentrale Aorta postoperativ radiologisch überprüft und ein Tumor im distalen Aortenbogen diagnostiziert. Der Tumor wurde reseziert und die Aorta durch ein Dacron-Interponat ersetzt. Die Histologie ergab ein Aortensarkom. Der postoperative Verlauf war unkompliziert, der Patient wurde in gutem Zustand nach Hause entlassen. Dieses Fallbeispiel belegt, dass es sich bei unklaren thrombotischen Verschlüssen lohnt, diese histologisch überprüfen zu lassen.
Abstract
A 63-year old man suffering from bilateral leg pain was admitted to our hospital with an occlusion of the infrarenal abdominal aorta. A bifurcated dacron graft was implanted and a histological examination of the thrombotic material was performed. Atypical cells were detected in the thrombus leading to an investigation of the thoracic aorta showing an unclear mass in the aortic arch. The tumor was resected and the aorta replaced with a dacron graft. Histology revealed a primary sarcoma. The patient is doing well 6 months after operation.
