Zusammenfassung

Seit der Entdeckung von Lithium zur Behandlung von psychischen Erkrankungen vor über 50 Jahren gibt es ein effektives Mittel zur Therapie von bipolaren Psychosen. Leider profitiert nur ein Teil der Patienten von dieser Substanz. Die geringe therapeutische Breite und die schlechte Verträglichkeit zum einen und die daraus resultierende fehlende Akzeptanz bei den Patienten verhindert in vielen Fällen den Einsatz von Lithium. Carbamazepin und Valproat waren die ersten Antikonvulsiva, die eine wichtige Bereicherung in der Pharmakotherapie von bipolaren Psychosen darstellen. Trotzdem gibt es noch einige Patienten, die auch auf diese Substanzen nicht ansprechen oder sie nicht vertragen. Neuere Antikonvulsiva, besonders Lamotrigin und Topiramat, konnten bereits in einigen Studien ihre Wirksamkeit zeigen. Auch die Kombination von Stimmungsstabilisierern könnte ein erfolgversprechender Therapieansatz sein. Insgesamt gibt es noch zu wenige Studien, um die Wirksamkeit dieser neuen Ansätze beurteilen zu können.

Abstract

For more than 50 years we have used Lithium as a very effective drug to treat bipolar disorders. Unfortunately only some patients took advantage of this treatment. Due to its small therapeutic range and the many side effects, many patients don’t accept this treatment. Carbamazepin and Valproat were the first two anticonvulsive drugs which enriched our therapeutic possibilities. But there are some patients left which are still not doing well. It seems like the newer anticonvulsants, especially lamotrigine and topiramate, could be another option in treating bipolar disorders. Both showed some promising study results. Combining two mood stabilisers may be another good way for treating severely ill patients. But we still need more investigations to make a final judgement on the value of these treatments.
