Zusammenfassung

Fragestellung

Die infiltrative Fibromatose der Brust ist eine sehr seltene, benigne Erkrankung, die lokal aggressiv destruierend wächst, aber nicht metastasiert. Klinisch, mammographisch und sonographisch imponiert sie wie ein Mammakarzinom.

Methodik

Es wird über eine 72-jährige Patientin mit einer infiltrativen Fibromatose der Brust berichtet.

Ergebnisse

Die Dignitätsvorhersage ist häufig erschwert und falsch positiv, wie auch die Feinnadelaspirationszytologie, Stanzbiopsie und eine intraoperative Schnellschnittuntersuchung.

Schlussfolgerung

Wir empfehlen ein zweizeitiges Vorgehen. Eine Schnellschnittuntersuchung ist wegen einer hohen Rate an falsch positiven Befunden nicht indiziert. Eine weiträumige Exzision im Gesunden ist aufgrund der hohen Rezidivneigung die Therapie der Wahl.

Abstract

We report on a 72-year-old woman with infiltrating fibromatosis of the breast. This is a rare, benign disease with aggressive and destructive local growth but without distant spread. Wide local excision is the therapy of choice because the local recurrence rate is high. The clinical, mammographic, and sonographic features of infiltrating fibromatosis are those of breast cancer. In our patient intraoperative frozen sections led to a false diagnosis of a malignant process and mastectomy was performed. Because of frequent false positive results we recommend a two-stage approach without frozen-section histology for patients with infiltrating fibromatosis of the breast.
