Behandlungsoptionen distaler metaphysärer Tibiafrakturen

Zusammenfassung

Frakturen der distalen Tibia mit und ohne Gelenkbeteiligung resultieren entweder aus einem „low energy“ oder einem „high energy trauma“ mit unterschiedlicher Pathologie. Die operative Behandlung hat dementsprechend die Frakturmorphologie und den oft erheblichen verletzungsbedingten Weichteilschaden zu berücksichtigen. Die klassische Plattenosteosynthese birgt ein hohes Risiko von Wundheilungsstörungen und wird zunehmend durch minimal-invasive perkutane Schraubenosteosynthesen mit oder ohne subkutan durchgeschobene Platten durchgeführt. Alternativ kann eine dorsale Plattenosteosynthese oder die Ausbehandlung im Hybrid-Fixateur zu einem zufriedenstellenden Ergebnis führen. Die Indikation und Durchführung der verschiedenen Behandlungsalternativen wird daher vergleichend dargestellt.

Abstract

Fractures of the distal tibia with or without joint destruction result from low or high energy trauma with different pathology, respectively. The treatment, therefore, needs to consider the various fracture types and the soft tissue injury. Open reduction with internal fixation of these fractures according to the AO principles needs using a wide incision is more and more replaced by minimal-invasive techniques. Percutaneus screw fixations or biological osteosynthese with subcutaneous plate insertion reduced soft tissue alterations. As an alternative an osteosynthesis with a dorsal plate or treatment with Hybrid-Fixator may be applied with good results. The different operative treatment options are demonstrated with their specific indications.
