Weiterbildungsziele

Schwerpunkt dieses Kapitels soll die Diskussion ausgewählter Kollagenosen und Vaskulitiden jeden Alters sein, die eine anästhesiologische und/oder intensivmedizinische Bedeutung erlangen können. Diese Auswahl musste wegen der großen Anzahl an Systemerkrankungen vorgenommen werden, da die hier nicht zitierten Krankheitsbilder keine bzw. nur eine unspezifische Bedeutung für den Anästhesisten, Intensiv- und Notfallmediziner haben.

Ätiologisch wird meist eine immunologische Ursache vermutet. Die Mehrzahl dieser Erkrankungen hat einen systemischen Charakter und kann jedes Organ betreffen. Darüber hinaus können die immunologisch induzierten Entzündungsreaktionen manchmal zu lebensbedrohlichen Situationen mit intensivmedizinischer Relevanz führen.

Die multifaktoriellen Therapieformen dieser Krankheitsbilder erfordern eine zeitgerechte präanästhesiologische Visite und erhöhte perioperative anästhesiologische Aufmerksamkeit. Dies bedeutet, dass der Patient neben den allgemeinen Risken über krankheitsbedingte Beatmungsprobleme, eine perioperative substitutionsbedürftige Anämie, mögliche Gerinnungsstörungen, ein erhöhtes Embolie- bzw. Infektionsrisiko, die anästhesiologischen Risiken durch Dauermedikation und deren Interaktionen sowie über eine notwendige perioperative Intensivtherapie zur Stabilisierung seiner Organdysfunktion aufgeklärt werden und der Anästhesist diese Aspekte beachten muss.
