Endoscopic Laser Coagulation of the Ciliary Processes in Patients with Severe Chronic Glaucoma

Hintergrund: Untersuchung der Wirkung der endoskopischen Zyklophotokoagulation der Ziliarapophysen zur Kontrolle des Augendrucks bei Patienten mit einem fortgeschrittenen chronischen Glaukom. Patienten und Methoden: 6 Augen (5 Pseudophakien, 1 Phakie) von 5 Patienten (Durchschnittsalter 60 Jahre, Variationsbreite 46 - 70 Jahre) wurden behandelt. Ursachen des Glaukoms waren Netzhautablösungsoperationen (Patient 1), Panuveitis (Patient 2), Irisdystrophie (Patient 3) oder Neovaskularisationen (Patient 4: Zentralvenenverschluss; Patient 5: proliferative diabetische Retinopathie). Bei allen Patienten war eine ungenügende präoperative antiglaukomatöse topische und systemische Behandlung vorhanden (4,2 Medikamente im Schnitt, Variationsbreite 3 - 5). Eine Trabekulektomie bei den Patienten 2 und 3 blieb erfolglos. Nach Pars-plana-Vitrektomie wurden die Ziliarapophysen unter endoskopischer Kontrolle mit einem Endolaser (Argon grün, Herde von 300 bis 500 mW à 0,4 - 0,5 s) über 180 - 270° koaguliert. Beide Augen von Patient 5 wurden behandelt. Ergebnisse: Der präoperative Durchschnittsaugendruck betrug 39 mm Hg (Variationsbreite 32 - 47). Die Beobachtungszeit betrug im Schnitt 339 Tage (Variationsbreite 125 - 485). Der postoperative Augendruck blieb über ein Jahr unter 21 mm Hg bei Patient 1 mit einem topischen Medikament und bei den Patienten 2 und 3 ohne zusätzliche Therapie. Eine zusätzliche Trabekulektomie und ein topisches Medikament waren notwendig, um den Augendruck von Patient 4 zu kontrollieren. Der Augendruck von Patient 5 mit dem kürzesten Follow-up wurde trotz zusätzlicher Therapien nie zufrieden stellend eingestellt. Es wurde keine schweren behandlungsbedingten Komplikationen festgestellt. Schlussfolgerungen: Die endoskopische Zyklophotokoagulation erlaubt eine genaue Destruktion der Ziliarapophysen und eine bessere Kontrolle des Augendrucks in gewissen Fällen von fortgeschrittenem chronischen Glaukom. Weitere Untersuchungen sind notwendig, um diese Therapie besser beurteilen zu können.

Abstract

Background: To investigate the effect of endoscopic laser coagulation on the ciliary processes in order to control intraocular pressure (IOP) in patients with severe chronic glaucoma. Patients and methods: 6 eyes (5 pseudophakic, 1 phakic) of 5 patients (mean age 60 years, range 46 - 70) were treated. Glaucoma was related to previous detachment surgery (patients 1), panuveitis (patient 2), iris dystrophy (patient 3), or neovascularization (patient 4: central venous occlusion; patient 5: proliferative diabetic retinopathy). Preoperatively, all patients had not responded to intensive glaucomatous topical and systemic treatment (mean 4.2 drugs, range 3 - 5). Trabeculectomy has been unsuccessfully performed in patients 2 and 3. After pars plana vitrectomy, the ciliary processes were coagulated under endoscopic view over 180 - 270° using endolaser (argon green with spots of 300 - 500 mW for 0.4 - 0.5 s). Patient 5 was treated in both eyes. Results: The mean preoperative IOP was 39 mm Hg (range 32 - 47). The mean postoperative follow-up was 339 days (range 125 - 485). The postoperative IOP was over 1 year under 21 mm Hg in patient 1 with one topical drug, and in patients 2 and 3 without any further treatment. Patient 4 needed trabeculectomy and one topical drug to control IOP. The IOP was not controlled in patient 5 with the shortest follow-up despite additional topical treatment. No serious treatment-related complication was noted. Conclusions: Endoscopic laser coagulation allows a precise destruction of the ciliary processes, and permits a better control of IOP in certain cases of severe chronic glaucoma. Further investigations are necessary to better evaluate this therapy.
