Die Differenzialindikation für die operative Versorgung des rheumatischen Schultergelenks

Zusammenfassung

Sonografische Befunde lassen die Befallshäufigkeit am Schultergelenk objektiver frühzeitig erkennen. Die Befallshäufigkeit ist daher nach einem Krankheitsverlauf von 2 Jahren in einem Bereich von 75 - 95 % anzusiedeln. Die Symptome des betroffenen Schultergelenks sind anfänglich schleichend und nur von gelegentlichen Schmerzattacken begleitet. Der Verlauf neigt aber zu einer raschen Progredienz mit Bewegungseinschränkung. Hauptschmerzursache ist der subakromiale Raum mit Beteiligung der Bursa subdeltoidea in 69 % der Fälle, der Bizepstendinitis in 75 % und der Supraspinatusläsion in 39 %. Das Ausmaß der Weichteilbeteiligung im Rahmen der Entzündung ist für das Schicksal des Schultergelenks von entscheidender Bedeutung. Nicht mehr rekonstruierbare Teilverluste der Rotatorenmanschette waren früher Kontraindikation für eine rekonstruktive Versorgung der Schulter. Einig sind sich im Wesentlichen alle Autoren heute darüber, dass Insuffizienz und Defekte der Rotatorenmanschette letztendlich Ursache schlechter Ergebnisse sind. Ferner wird eine rekonstruktive Versorgung des Schultergelenks durch die Knochenqualität beeinflusst. Knöcherne Defekte lassen oftmals nur noch begrenzt endoprothetische Maßnahmen zu. Die differenzialdiagnostischen und therapeutischen Möglichkeiten werden diskutiert.

Abstract

Sonographic findings show the involvement of the shoulder joint by rheumatoid arthritis very early. After 2 years, already 75 to 95 % of the shoulders are definitely affected. The symptoms are slowly increasing and only occasionally accompanied by pain attacks. Progression leads to rapidly decreasing range of motion and loss of strength. The main pathology occurs in the subacromial space with inflammatory changes in the bursa, supraspinatus tendon and biceps tendon. The amount of soft tissue destruction determines the fate of the shoulder joint. Partial rotator cuff tears that could no longer be repaired were seen as contraindication for reconstructive surgery in the past. Agreement exists today that insufficiency and defects of the rotator cuff lead to early loosening of glenoid replacements and are responsible for bad results. Furthermore, the quality of bone often limits any endoprosthetic joint replacement of the shoulder. Differential diagnostic and therapeutic aspects are discussed.
