Zusammenfassung
Hintergrund: Das aggressive Angiomyxom (AAM) ist ein seltener Weichgewebstumor, der vorwiegend in der Becken- und Genitalregion prämenopausaler Frauen auftritt. Es handelt sich um einen langsam und lokal infiltrativ wachsenden Tumor, der zu Lokalrezidiven neigt.
Kasuistik: Wir berichten über den Krankheitsverlauf einer 30-jährigen Patientin, bei der über einen Zeitraum von 4 Jahren das Wachstum eines pelvinen Tumors exspektativ behandelt wurde, bevor sie an ein Zentrum zur definitiven Therapie überwiesen wurde. Während dieser Zeit wurde anfangs die Fehldiagnose einer Gardner-Gangszyste, später einer Fibromatose gestellt. Mehrere chirurgische Interventionen erbrachten jedoch keine komplette Tumorreduktion. Im Juli 2002 führten wir eine Längsschnittlaparotomie mit makroskopisch kompletter Tumorresektion, unter Erhalt der Fertilität, durch. Histologisch handelte es sich um ein aggressives Angiomyxom. Der postoperative Verlauf gestaltete sich komplikationslos, die Patientin berichtet über eine regelmäßige Zyklusanamnese und über ein beschwerdefreies, aktives Sexualleben.
Schlussfolgerung: Ungewöhnliche Krankheitsverläufe, bei denen über einen Zeitraum von 4 Jahren ein Tumorwachstum beobachtet wird, sollten unbedingt vermieden werden. Zur Verbesserung der Therapiestrategien bei gynäkologisch seltenen Tumoren wäre die Einrichtung eines bundesweiten Tumorregisters hilfreich; die Behandlung seltener gynäkologischer Tumoren sollte in spezialisierten Zentren stattfinden.
Abstract
Background: Aggressive angiomyxoma (AAM) is a rare semi-malignant tumor of the soft tissue predominantly occurring in the genital and pelvic area. It tends to occur more frequently in premenopausal women and it is characterized by slow growth and local infiltrative aggressiveness. It has a considerable risk for local recurrences.
Patient and Method: We report the case of a 30-year-old female who had a pelvic tumor for a period of 4 years without adequate therapy before attending our hospital. During this period, the tumor was misdiagnosed first as a Gardner-cyst, later as fibromatosis. Several surgical attempts did not result in complete resection. In July 2002 we performed a laparotomy and complete tumor reduction, preserving fertility potential. Histological examination revealed an aggressive angiomyxoma. The patient remained well after operation and no menstrual or coital problems occurred.
Conclusion: An extended case history of progressive tumor growth should not take 4 years to diagnose. A national registry should be established to aid the diagnosis of rare gynecological tumors. The treatment of such tumors should take place in specialized centers.
