Entlang der linea anaesthetica: Ein neuer Zugang zum Plexus brachialis - die subcoracoidale retrograde Plexuspunktion (SCREP)

Zusammenfassung

Fragestellung: Vorgestellt wird ein neuer Zugang zum Plexus brachialis, der sich gegenüber den herkömmlichen Methoden durch zusätzliche theoretische Vorteile auszeichnet. So erfolgt die Einstichsrichtung in spitzem Winkel zum Plexusverlauf mit einer geringeren Gefahr der Nervenschädigung. Daher ist auch eine Kathetereinlage erleichtert. Daneben sind Pleurapunktionen, abgesehen von groben Abweichungen vom Punktionsschema, nahezu unmöglich. Zusätzlich wird der Plexus vor Abgang des N. musculocutaneus erreicht mit einer gewissen Relevanz für die operative Versorgung von Radiusfrakturen. Ziel dieser Arbeit war es zu evaluieren, inwieweit sich die subcoracoidale retrograde Plexuspunktion im täglichen Routinebetrieb bewährt. Daneben wurde der Frage nachgegangen, ob eine Faszikelpräferenz oder eine bestimmte Mindeststimulationsstärke einen nachweisbaren Einfluss auf die Trefferquote hat. Methodik: An 75 Patienten, die für einen operativen Eingriff an der oberen Extremität vorgesehen waren, wurde eine Plexusanästhesie mit subcoracoidaler retrograder Punktion nach einem vorgegebenen Schema durchgeführt. Abweichungen vom Schema, stimulierter Faszikel sowie erreichbare Mindeststimulationsstärke wurden protokolliert, ebenso wie Wirkbeginn und Zeitpunkt der komplett ausgebildeten Plexusblockade. Vorhandene Defizienzen wurden ebenfalls erfasst und der Block als Erfolg oder Nichterfolg klassifiziert. Zum Nachweis einer Faszikelpräferenz wurden die für die einzelnen Faszikel gefundenen Erfolgsquoten dem chi²-Unabhängigkeitstest unterzogen. Zum Nachweis, inwieweit eine bestimmte Mindeststimulationsstärke die Trefferquote beeinflusst, wurden die Streu- und Mittelwerte der Stimulationsstärken für die einzelnen Faszikel dem Stichprobentest nach Kolmogorov-Smirnow sowie dem Zweistichprobentest Z unterzogen.

Ergebnisse: Es konnte eine Trefferquote von 92 % erreicht werden, insbesondere fanden sich keine Defizienzen im Versorgungsbereich des N. cut. antebrachii lateralis. Durch die retrograde Injektion des Lokalanästhetikums fand sich zusätzlich in einem hohen Prozentsatz eine Anästhesie des Schulterbereichs. Die Komplikationsrate war mit 3 % niedrig und betraf jeweils Gefäßpunktionen. Anhand der 75 in die Arbeit eingegangenen Fallbeobachtungen konnte aufgezeigt werden, dass weder eine Faszikelpräferenz noch eine bestimmte Mindeststimulationsstärke einen signifikant nachweisbaren Einfluss auf die Trefferquote hat. Schlussfolgerung: Der beschriebene subcoracoidale retrograde Plexuszugang (SCREP) stellt eine sichere, komplikationsarme, leicht reproduzierbare und mit einer hohen Erfolgsquote behaftete Alternative im Spektrum der bekannten praktizierten Methoden dar. In Abgrenzung zu anderen Empfehlungen konnte gezeigt werden, dass die Stimulation eines bestimmten Faszikels bei der vorgestellten Punktionsmethode keinen Einfluss auf die Erfolgsquote hat, die ebenso nicht durch die Stimulation unterhalb definierter Stimulationsstärken positiv beeinflusst wird.

Abstract

Objective: Introduced is a new approach to the brachial plexus marked out in opposition to the common methods by some theoretical advantages. The needle is proceeded in an acute angle to the course of the plexus. Thus reduces the possibility of nerve damage. Second the insertion of catheter is simplified. The accidental puncture of the pleura is unlikely except in cases of gross deviations of the puncture instruction. In addition the plexus is hit before the musculocutaneous nerve leaves the brachial plexus. The objective of this investigation was to evaluate the usefulness of the subcoracoidal retrograde access in daily practice. Beside the influence of stimulating different fascicles or the necessity of reaching a defined low stimulation level on the success rate and effectiveness of the plexus blockade was investigated. Methods: 75 patients undergoing elective surgery in upper limb received plexus anesthesia with the subcoracoidal retrograde access. Necessary deviation of the planned direction of puncture, stimulated fascicle, the reached minimum level of stimulation, the onset of anesthesia, point of time of the complete anesthesia were recorded. Deficiencies were also recorded and registered as sufficient or insufficient nerve blockade. The success rate of the stimulation of the different fascicles were evaluated by the chi²-Test to find out a preference a certain fascicle. The deviation and medium of the lowest found current levels for the different fascicles were evaluated by the Kolmogorov-Smirnow Test and the Two Sample Test Z to show to what extent a certain lowest current level influences the success rate. Results: The overall success rate was 92 %, particularly the anesthesia in the region of the lateral cutaneous nerve of the forearm was achieved in all cases. As a side effect caused by the retrograde injection of the local anesthetic anesthesia of the shoulder was also achieved with a high frequency. The low complication rate of 3 % consists only in accidental puncture of vessels. We found no significant influence of stimulating a certain fascicle or reaching a defined minimum stimulation level on the success rate of nerve blockade. Conclusion: The described subcoracoidal retrograde access to the brachial plexus (SCREP) proved to be a safe and easy method with a low complication and high success rate compared with the known practised accesses. In contrary to other authors our findings do not support the connection either between success rate and stimulating certain fascicles nor achieving a defined lowest stimulation level.
