Reproduktion nach Mammakarzinom - Was können wir unseren Patientinnen raten?

Zusammenfassung

Der Kinderwunsch wird heute später realisiert, daher haben viele Frauen bei der Diagnose Mammakarzinom ihre Familienplanung noch nicht abgeschlossen. Der folgende Artikel befasst sich mit der Thematik Reproduktion nach und trotz Mammakarzinom. Durch die Polychemotherapien in der Adjuvans werden durchschnittlich 50 % der Frauen amenorrhoisch. Es gibt noch keine wissenschaftlich abgesicherte Methode die Fertilität zu erhalten. Retrospektive Untersuchungen und Fall-Kontrollstudien konnten zeigen, dass das Risiko eines Rezidivs durch eine nachfolgende Schwangerschaft nicht erhöht ist. Es gibt jedoch keinen „besten” Zeitpunkt für die Schwangerschaft. Dieser muss individuell unter Berücksichtigung der Prognose, dem Alter und der eventuell noch durchgeführten Hormontherapie festgelegt werden. Von einer Schwangerschaft sollte man jedoch in keinem Falle abraten. In wie weit bei sich nicht spontan einstellender Schwangerschaft assistierte reproduktive Verfahren (ART) eingesetzt werden können, ist bisher ungeklärt. Umgekehrt gibt es aber auch keine Hinweise, dass eine ART die Prognose verschlechtert.

Abstract

Nowadays women delay their childbirth to the 30ies. Therefore, more breast cancer patients haven't completed their family planning and want to get children after the diagnosis of breast cancer. Because of anthracycline and cyclophosphamide containing polychemotherapies for the adjvuant treatment of breast cancer, about 50 % of the patients will be ammenorrhoic after finishing treatment. So far there are no valid treatment options for preserving ovarian function after chemotherapy. There is no increased risk for relapse if a breast cancer patient becomes pregnant. However, the timing of the pregnancy has not yet been fixed. It depends on the prognosis, the age, the personal situation and personal preferences.
