Zusammenfassung
Ziel der Studie: Die orthotope Lebertransplantation (LTX) ist ein etabliertes Standardtherapieverfahren von Endzuständen verschiedener Lebererkrankungen. Da für dieses operative Verfahren und dessen anästhesiologisches Management noch keine Therapiestandards vorliegen, war es das Ziel der vorliegenden Untersuchung, den aktuellen Stand der anästhesiologischen Vorgehensweise bei diesem Operationsverfahren zu erfassen. Methodik: Es wurden alle mit Lebertransplantationszentren zusammenarbeitende anästhesiologische Kliniken in Deutschland angeschrieben (n = 24). Zu folgenden Komplexen wurden Daten erhoben: Anästhesieverfahren, Anästhetika, Blutkomponententherapie, perioperatives Monitoring, kardiovaskuläre Supportivtherapie, Organisations- und Personalstruktur u. a. Ergebnisse: Aus den bis zum 31. 1. 2004 eingegangenen Rückantworten lässt sich folgender Trend ableiten: Das Anästhesieverfahren der Wahl stellt die balancierte Anästhesie (80 %) mit kontinuierlicher Opioidapplikation dar. Dabei werden sowohl unterschiedliche volatile Anästhestika (Isofluran mit 66,7 %) favorisiert als auch verschiedene Opioide eingesetzt. Fentanyl (53,3 %) wird dabei bevorzugt. Der venovenöse Bypass wird bei 86,7 % der antwortenden Kliniken nie oder nur noch gelegentlich verwendet. Die in den letzten Jahren geführte Diskussion zum differenzierten Einsatz von Katecholaminen spiegelt sich auch in dieser Untersuchung wider. Dabei erwies sich Dobutamin/Noradrenalin als die Kombinationstherapie der ersten Wahl (46,7 %). Während über die Bereitstellung von Blutkomponenten in der Mehrzahl der Kliniken Übereinstimmung bestand, variierte die Anzahl der applizierten Transfusionseinheiten stark. Die perioperative Gabe des Proteinaseinhibitors Aprotinin erfolgte in 60 % der Kliniken gelegentlich, in 20 % der Kliniken immer und in weiteren 20 % der Kliniken nie. In der überwiegenden Zahl der Kliniken erfolgt die intraoperative Betreuung durch > 2 Anästhesisten (80 %) und 1 Anästhesieschwester (53,3 %). Bezüglich der Nachbetreuung werden die Patienten sowohl in anästhesiologisch als auch in chirurgisch geleiteten Intensivstationen versorgt. Es wird allgemein angestrebt, die Patienten nach Eintreffen auf der Intensivstation im Sinne eines Fast-Track-Verfahrens möglichst frühzeitig zu extubieren (86,7 %). Schlussfolgerung: Wenngleich Übereinstimmungen in der anästhesiologischen Versorgung während einer LTX herausgearbeitet werden konnten, gibt es auch differenzierte Vorgehensweisen. Die Daten können und sollen zur kritischen Verfahrensdiskussion und Erarbeitung von anästhesiologischen und intensivtherapeutischen Standards für das untersuchte Operationsverfahren führen.
Abstract
Objective: Since there is no therapeutical standard for the anaesthesiological approach during liver transplantation (LTX) in Germany at the moment, we have evaluated the current anaesthesiologic procedures during LTX. Methods: All departments of anaesthesiology (n = 24) cooperating with transplantation centers in Germany received a questionnaire via mail regarding following complexes: anaesthesiological methods, anaesthetics, blood components therapy, perioperative monitoring, supportive cardiovascular therapy and staff. Results: The answers (n = 16) showed following results: Balanced anaesthesia with continuous application of opioids was the standard method (80 %). Different volatile anaesthetics as well as different opioids were used, isoflurane (66.7 %) and fentanyl (53.3 %) were the most common. Veno-venous bypass was never or occasionally used (86.7 %). The differentiated use of catecholamines, based on discussions in the last years, was also reflected in the results. Dobutamine/noradrenaline as combination seemed to be the first choice (46.7 %). Whereas there was an accordance with the employment of blood components, there was a large variation in the effectively applicated blood products. Aprotinin was given in 60 % of all clinics occasionally, in 20 % everytime and in 20 % aprotinine was never used. In most departments ≥ 2 anaesthesiologists (80 %) and 1 nurse (53,3 %) were needed for intraoperative observation. Postoperative medical attendance was provided on anaesthesiological as well as surgical guided intensive care units (ICU). Generally accepted was an early extubation after arrival at the ICU (86.7 %). Conclusion: Even though there was a consensus in the anaesthesiological approach during LTX some departments still use different procedures. This is the first study that will give a basis for discussion of anaesthesiological approaches during LTX.
