Die hochauflösende Sonographie des Gastrointestinaltrakts hat heute einen festen Platz im diagnostischen Arsenal der Gastroenterologie. Dies war keinesfalls immer so, wurde doch lange der GI-Trakt mehr als störendes Organsystem betrachtet: Luft- und Chymusanteile erschwerten die Sonographie anderer Organe oder machten sie gar unmöglich.
Nach ersten experimentellen Studien von Wild und Reid Anfang der 50er Jahre veröffentlichte H. Lutz 1972 auf dem Weltkongress für Gastroenterologie erstmals das Bild einer zirkulären Magenwandverdickung bei einem submukös wachsenden szirrhösen Karzinom, der Linitis plastica (Abb. [1]). Im Querschnitt wurde für dieses typische Erscheinungsbild des Tumors der Begriff der "Kokarde" geprägt.

Der Begriff wurde auch für andere Abschnitte des GI-Trakts verwendet, so z. B. die Kolonkokarde bei fortgeschrittenen Kolonkarzinomen, wie ebenfalls bereits 1972 von H. Lutz beschrieben. Aber auch nichtneoplastische Erkrankungen können zum (zunächst rein deskriptiven) Bild einer Kokarde führen, beispielsweise die Minikokarde bei der Appendizitis oder die "Kokarde in der Kokarde" bei der Invagination. Andererseits erscheint es wenig sinnvoll den Begriff auch für Anschnitte des normalen GI-Traktes zu verwenden ("physiologische Kokarde"), da er beim Unerfahrenen einen pathologischen Befund impliziert.
Mit Verbesserung der Gerätetechnik ließen sich Mitte der 80er Jahre auch umschriebene Tumoren des GI-Traktes sonographisch nachweisen. Abb. [2] zeigt ein Karzinom des Magenkorpus bei gefüllten Magen.

Die Untersuchungstechnik wurde durch die Wasserkontrastmethode (Worlicek 1986) verfeinert, die auch die mittleren Magenabschnitte mittels einer besonderen Lagerungstechnik beurteilen lässt, sowie die Zuordnung umschriebener Veränderungen zum Magen erlaubt. Endoskopisch suspekte Befunde wie Impressionen oder fehlende Entfaltung des Magens lassen sich so vielfach bereits transkutan weiter abklären, insbesondere wenn kein Gerät für die EUS zur Verfügung steht.
Auch nach der Entwicklung der Endosonographie im oberen Verdauungstrakt (ab 1978) bleibt die transkutane Burteilung des Magens Bestandteil einer gründlichen Ultraschalluntersuchung. Jeder Zufallsbefund einer Magenwandverdickung bedarf der weiteren endoskopischen Abklärung (Abb. [3]). Insgesamt gilt eine Wanddicke >6mm als pathologisch, wobei allerdings Fehler bei leerem Magen infolge der zusammenliegenden Falten entstehen können.

Andererseits haben wir heute gelernt, dass auch zahlreiche andere Erkrankungen zu zirkulären Wandverdickungen des Magens führen können: Lymphome, schwere Gastritis, portalhypertensive Gastropathie, ... . Abb. [4] zeigt als Beispiel die ausgeprägte Magenwandverdickung i.R. der entzündlichen Mitreaktion bei nekrotisierender Pankreatitis.

Transcutaneous Sonography of the Stomach
High-resolution sonography of the gastrointestinal tract has become a standard procedure in gastroenterological diagnostics. This was not always the case, as the intestinum was more or less considered to be an obstacle to sonographic examination, with gas and intestinal content impairing visualisation of other organs or rendering the examination impossible altogether.
In the aftermath of first experimental studies by Wild and Reid at the beginning of the fifties, H. Lutz was the first to publish the image of a circular enlargement of the stomach wall due to a submucosal scirrhous tumour, termed "Linitis plastica (leather bottle stomach)", at the World Congress of Gastroenterology in 1972 (Fig. [1]). The typical crosssectional image of this tumour was then termed the "cockade sign".
This term was also used for other parts of the gastrointestinal tract, one example being the cockade appearance of an advanced carcinoma of the colon, also described by H. Lutz in 1972. Non-neoplastic diseases, however, can also appear in the form of a cockade, which basically is just a descriptive term. Examples of this are the mini-cockade of appendicitis or the "cockade within the cockade" in cases of intussusseption. It is not very sensible to use the term "physiological cockade", as this might suggest a pathological finding to the inexperienced examiner.
The technical improvement of ultrasound apparatus facilitated the detection of circumscribed tumours of the gastrointestinal tract in the mideighties. Fig. [2] demonstrates a carcinoma of the gastric corpus in a full stomach.
Examination technique was refined by the water contrast method (Worlicek 1986), which, with the help of a specific positioning technique, allowed scanning of the central parts of the stomach as well as ascribing circumscribed lesions to the stomach. Endoscopically suspicious findings, as there are indentations of the gastric wall or failure of the stomach to unfold, can often be diagnosed by a transcutaneous approach, especially in those cases where endosonography is not available.
Transcutaneous examination of the stomach should still be part of a thorough ultrasound check even after the advent of endosonography of the upper gastrointestinal tract (from 1978). Any incidental finding of a thickened gastric wall must be further examined by endoscopy (Fig. [3]). A stomach wall of >6mm in diameter is considered to be pathological, as long as the possibility of closely packed folds in an empty stomach is kept in mind.
We have also come to acknowledge the fact that a number of different diseases can cause circular thickening of the stomach wall, as there are: lymphomas, severe gastritis, gastropathy in portal hypertension, ... . Fig. [4] demonstrates an example of pronounced swelling of the stomach wall as a reactive feature in a case of necrotising pancreatitis.
