Antibiotische Behandlung einer Implantat-assoziierten Endokarditis mit Fosfomycin und Vancomycin bei Patienten mit Niereninsuffizienz

Infektiöse Endokarditis als Folge einer Implantation einer künstlichen Herzklappen-prothese oder eines Schrittmachers ist eine therapeutisch komplexe Erkrankung mit hoher Letalität. Meist werden die Implantate von gram-positiven Kokken (Staphylokokken) besiedelt [1]. Da diese häufig gegen Penicilline resistent sind, wird eine Kombinationstherapie mit Vancomycin und Gentamicin empfohlen [3]. Diese Antibiotikatherapie führt in bis zu 38 % der Fälle zu Niereninsuffizienz. Dazu kommt, dass mit steigender Tendenz ältere, niereninsuffiziente Patienten mit Prothesen am Herzen versorgt werden.

Es wurde schon seit langem versucht, diese Kombination durch weniger toxische zu ersetzen. Im Tiermodell sowie bei nierentransplantierten Patienten wurde die Kombination Vancomycin plus Fosfomycin untersucht und dabei der überraschende Befund erhoben, dass die Kombination weniger nephrotoxisch war als Vancomycin allein. Wahrscheinlich verhindert Fosfomycin die Akkumulation von Vancomycin in der Niere [2] [4].

Wir präsentieren fünf Patienten, die mit Vancomycin und Fosfomycin behandelt wurden.
