Zusammenfassung
Der Paradigmenwandel in der Onkologie mit seinem Reichtum an neuen, teils nicht zytotoxischen Medikamenten zeigt Auswirkungen auch auf die Palliativmedizin. Im Setting einer spezialisierten Palliativstation können sich durch internistisch-onkologisches Zutun durch diese Entwicklung neue symptomgerichtete Therapieoptionen ergeben. Es ist abzusehen, dass der Beratungsbedarf von Patienten bezüglich der neuen „Wundermedikamente” zunehmen - und auch an die Mitarbeiter einer Palliativstation gerichtet - werden wird. In dieser Übersicht sollen zunächst mögliche Implikationen dieser Entwicklung auf primär palliativmedizinische Therapiekonzepte zur Diskussion gestellt werden. Die individuell abzuwägende Entscheidungsfindung zu einer onkologisch initiierten und begleiteten tumorspezifischen Therapie als ergänzende, symptomorientierte Maßnahme auf einer Palliativstation wird erörtert. Der dritte Abschnitt gibt eine Übersicht über die zur Verfügung stehenden Substanzen, die für den Gebrauch auf einer nicht onkologischen Palliativstation relevant sind; konventionelle zytotoxische Medikamente oder neue Substanzen, die aufgrund der Komplexität ihrer Applikation in die Handhabe einer internistisch-onkologischen Therapiestation gehören, werden der Vollständigkeit halber kurz genannt.
Abstract
Recent developments in medical oncology with numerous new anticancer substances will influence palliative care. Inpatient palliative care units might be faced with extended treatment options for symptom relief; and the needs of patients and their relatives, seeking medical advice concerning „new wonder pills”, might increase. This review article will discuss altered implications of oncology in genuine palliative care concepts and describes oncological considerations before initiating and escorting systemic anticancer therapy on a specialized palliative care unit. An overview will be given on newly developed cytotoxic agents, „targeted” non-cytotoxic substances and their mode of action. Established substances and those with complex application schedules or side effects will be briefly mentioned for the sake of completeness.
