Eine 85-jährige Patientin stellte sich mit einer seit 9 Monaten persistierenden Heiserkeit und aphonen Stimmlage beim Hals-Nasen-Ohren-Arzt vor. Es zeigte sich eine Stimmbandparese links, als deren Ursache sich im Röntgen Thorax ein ca. kindfaustgroßer Tumor im linken Mediastinum herausstellte. Die weitere Diagnostik zeigte ein 7 cm weites teilthrombosiertes thorakales Aortenaneurysma der Aorta descendens, dessen Beginn unmittelbar dem Abgang der Arteria carotis communis und der A. subclavia gegenüber liegt (Abb. [1]-[6]).

Eine Dilatation der Aorta über das 1,5-Fache der Altersnorm oder 3,5 cm für die Aorta thoracica wird als Aneurysma bezeichnet. Die Arteriosklerose ist die häufigste Ursache aneurysmatischer Veränderungen. Die transthorakale Echokardiographie erlaubt lediglich die Beurteilung der Aorta ascendens, nur bei guter Schallbarkeit kann auch der Aortenbogen dargestellt werden. Die ungewöhnliche Größe des Aneurysmas an der vorderen Thoraxwand erlaubte ausnahmsweise eine exzellente Darstellung. Häufig sind thrombotische Veränderungen nachzuweisen, wobei in unserem Fall neben einem echoreichen, zentral durchflossenen Thrombus zur Wand hin echoärmere, in Organisation befindliche Thromben, ähnlich "Wachstumsringen" darzustellen waren. Abhängig von der Ausgangsgröße werden in der Literatur Wachstumsraten von 0,5-1,0 cm/Jahr beschrieben.
Eine Operationsindikation ist ab einem Diameter von 5,5 cm gegeben, bei Überschreiten dieses Grenzwertes steigt die Rupturgefahr erheblich an. Aufgrund der Größe des Aneurysmas lag bei unserer Patientin eine klare Operationsindikation vor, unter Verweis auf ihr Alter lehnte die Patientin einen Eingriff vehement ab. Die Alternative einer Endoprothesenimplantation entfiel, da der Ursprung des Aneurysmas die Abgänge von linker A. carotis interna und A. subclavia miteinbezog. Der Patientin konnte somit nur eine konsequente Blutdrucksenkung angeboten werden, Als Therapie der Wahl gilt die Betablockergabe aufgrund der Verminderung der Wandspannung. Wenige Tage nach der Vorstellung in unserer Klink wurde die Patientin tot in ihrer Wohnung vor einem geöffneten Fenster aufgefunden. Als Todesursache ist bei der nicht obduzierten Patientin eine Aneurysmaruptur anzunehmen, offensichtlich hatte die Patientin bei akuter Dyspnoe noch versucht, durch Öffnen des Fensters Linderung zu bekommen.

Aneurysm of the Thoracic Aorta - Three Sections of the Aorta in a Single Scan
An 85-year-old patient consulted an ENT-specialist because of persisting hoarseness and an aphonic voice for the last 9 months. A paralysis of the left vocal cord could be found, due to a tumour the size of a child's fist on chest x-ray. Further diagnostic procedures demonstrated a partially thrombosed aneurysm of the ascending aorta, measuring 7 cm, commencing directly opposite the outlet of the common carotid and the subclavian artery (Fig. [1]-[6]).
The dilatation of the aorta of more than 1,5 times the age-adjusted norm, or 3,5 cm of the thoracic aorta, is defined as an aneurysm. Arteriosclerosis is the most common cause of aneurysmatic changes. Transthoracic echocardiography only allows the assessment of the ascending aorta, and the aortic arch can only be demonstrated under conditions optimal for sonographic examinations. The unusual size of this aneurysm close to the frontal chest wall facilitated the exceptional quality of the sonographic imaging. Signs of thrombosis can often be visualised, and in our case there was an echogenic, centrally perfused thrombus, and several other hypoechoic thrombi, showing signs of organisation, similar to "growth rings". Reports in the literature assume a growth rate of 0,5-1cm/year, depending on the original size.
Surgery should be considered when the diameter exceeds 5,5cm, as the danger of rupture increases dramatically beyond this value. In the case of our patient, the size of the aneurysm meant a clear indication for surgery, but the patient vehemently declined because of her age. The alternative option of implanting an endoprosthesis was not viable, since the aneurysm originated in the area of the outlet of the left internal carotid and the subclavian artery. The only viable alternative was therefore a stringent lowering of the blood pressure level. A beta-blocker is the medication of choice due to its property of reducing tension of the vessel wall. Several days later the patient was found dead in her apartment in front of the open window. She had probably experienced the rupture of the aneurysm and had tried to open the window in a state of acute dyspnea in order to get more air. The cause of death could not be confirmed, since a post-mortem examination was not carried out.
