Ulnares Sesambein des Kleinfingers als ungewöhnliche Ursache eines schmerzhaften schnellenden Fingers

Zusammenfassung

Wir berichten über den ungewöhnlichen Fall einer 38-jährigen Patientin, die unter dem Bild eines schmerzhaften schnellenden Kleinfingers revidiert wurde. Entgegen zu erwartender Befunde in Form eines verdickten Ringbandes beziehungsweise eines Ganglions auf der Beugesehnenscheide fanden sich intraoperativ faserartige Zügel zwischen einem ulnaren Sesambein und dem A1-Ringband, die eine seitliche Deviation der gesamten Vagina fibrosa mit konsekutiver Schnappsymptomatik bewirkten. Ein Zusammenhang zwischen Sesambein und Symptomatik eines schnellenden Fingers ist bisher mehrfach am Daumen beobachtet worden; für einen dreigliedrigen Finger wurde dieses Phänomen bisher nicht beschrieben.

Abstract

We report on a 38-year-old woman suffering from painful trigger finger. Contrary to the expected intraoperative finding of a simple stenosing pulley and ganglion cyst on a thickened flexor tendon sheath, we found fibrotic cords between an abnormal ulnar sesamoid bone at the fifth finger and the A1-pulley to be responsible for distortion of the tendon sheath and a consecutive “klicking”-phenomenon. A coherence between sesamoid bones and trigger finger has repeatedly been found on the thumb but there is no such description regarding the long fingers.
